Invisible orthodontic aligners: a review

Resumo: | Orthodontics is a specialty that assesses the growth and development of the face and teeth, preventing and correcting dental malocclusions up to deviations from normal face. Orthodontic treatment causes changes in aesthetics and function, thus impacting patients' quality of life. The existing protocols for tooth movement consist of the traditional fixed methods using brackets (metallic, plastic or ceramic) and metallic wires, and the invisible removable braces, made of thermoplastic material that present several benefits, such as their aesthetics and facilitating oral hygiene. Thus, this work has the general objective of conducting a literature review of the studies and research carried out in relation to invisible orthodontic appliances and to evaluate the available evidence on the clinical effectiveness of the system. 76 studies were analyzed. It was observed by the review analysis of the articles presented in the study that the system of invisible and removable appliances applied to orthodontic treatment has numerous advantages, therefore, this system has become increasingly demanded and chosen as an option for orthodontic treatment. It is concluded that advances in the quality of aligner materials, accessories and complementary systems, in the use of digital systems for processing initial data, as well as in virtual three-dimensional planning have expanded the range of possibilities of orthodontic treatment with invisible and removable appliances applied to various orthodontic problems, whether of lesser or greater complexity. The system has been widely used and its effectiveness, precision and predictability in treatment are observed in the studies. |

El tratamiento de ortodoncia provoca cambios en la estética y la función, afectando así la calidad de vida de los pacientes. Los protocolos existentes para el movimiento dentario consisten en los métodos tradicionales fijos mediante brackets (metálicos, plásticos o cerámicos) y alambres metálicos, y los brackets invisibles removibles, fabricados en material termoplástico que presentan varios beneficios, como su estética y facilitar la higiene bucal. Así, este trabajo tiene como objetivo general realizar una revisión de la literatura de los estudios e investigaciones realizadas en relación a la aparatología de ortodoncia invisible y evaluar la evidencia disponible sobre la efectividad clínica del sistema. Se analizaron 76 estudios. Se observó mediante el análisis de revisión de los artículos presentados en el estudio que el sistema de aparatología invisible y removible aplicado al tratamiento de ortodoncia tiene numerosas ventajas, por lo que este sistema se ha vuelto cada vez más demandado y elegido como una opción para el tratamiento de ortodoncia. Se concluye que los avances en la calidad de los materiales de alineadores, accesorios y sistemas complementarios, en el uso de sistemas digitales para el procesamiento de datos iniciales, así como en la planificación virtual tridimensional han ampliado el abanico de posibilidades del tratamiento de ortodoncia con invisibles y removibles. Aparatos aplicados a diversos problemas de ortodoncia, ya sean de menor o mayor complejidad. El sistema ha sido ampliamente utilizado y en los estudios se observa su efectividad, precisión y predictibilidad en el tratamiento.A Ortodontia é uma especialidade que avalia o crescimento e desenvolvimento da face e dos dentes, prevenindo e corrigindo as más oclusões dentárias até os desvios de normalidade da face. O tratamento ortodôntico causa alterações da estética a função impactando assim na qualidade de vida dos pacientes. Os protocolos existentes para movimentação dentária consistem nos métodos tradicionais fixos com uso de bráquetes (metálicos, plásticos ou cerâmicos) e fios metálicos, e os aparelhos removíveis invisíveis, confeccionados de material termoplástico que apresentam diversos benefícios, como sua esteticidade e facilitação da higiene bucal. Desta forma, este trabalho tem como objetivo geral realizar uma revisão de literatura dos estudos e pesquisas realizadas com relação aos aparelhos ortodônticos invisíveis e avaliar as evidências disponíveis sobre a eficácia clínica do sistema. Foram analisados 76 estudos. Observou-se pela análise de revisão dos artigos apresentada no estudo que o sistema de aparelhos invisíveis e removíveis aplicados ao tratamento ortodôntico apresentam inúmeras vantagens, por isso, este sistema tem se tornado cada vez mais demandado e escolhido como opção de tratamento ortodôntico. Conclui-se que os avanços na qualidade dos materiais do alinhador, acessórios e sistemas complementares, na utilização de sistemas digitais de processamento dos dados iniciais, bem como no planejamento tridimensional virtual expandiram a gama de possibilidades do tratamento ortodôntico com aparelhos invisíveis e removíveis aplicados a diversos problemas ortodônticos, seja de menor ou maior complexidade.
