Hemorrhagic stroke in a dog: case report

Resumo: Stroke is classified as ischemic or hemorrhagic and is an uncommon disease in small animals. This alteration progresses with nonspecific neurological signs and, in severe cases, rapid evolution to death, being difficult to diagnose antemortem. Our objective is to report a case of dog with an hemorrhagic stroke. A 2-year-old female dog was attended, presenting loss of appetite, apathy, blindness and ataxia, during the last 24 hours. During clinical evaluation, total bilateral blindness, fecal and urinary incontinence, muscular hypotonia and semicomatous state were observed. The animal quickly progressed to death. At necropsy, a clot was observed in the brain, measuring 6 cm length, 4 cm width and 2.5 cm height, reaching the lateral ventricles and third ventricle, associated with a probable rupture of choroid plexus vessels. Clinical signs were nonspecific, suggesting alterations in the neurological system, however, due to the rapid evolution, it was not possible to performed additional tests or treatment. Necropsy was essential to obtain a definitive diagnosis of hemorrhagic stroke.

Esta alteración conduce a signos neurológicos inespecíficos y, en casos graves, a una rápida evolución a la muerte, lo que dificulta el diagnóstico ante mortem. Nuestro objetivo es reportar el caso de un canino con accidente cerebrovascular hemorrágico. Se atendió una perra de 2 años, con pérdida de apetito, apatía, ceguera y ataxia en las últimas 24 horas. Durante la evaluación clínica se observó ceguera total bilateral, incontinencia fecal y urinaria, hipotonía muscular y estado semicomatoso, y el animal progresó rápidamente a la muerte. En la necropsia se observó en el encéfalo un coágulo de 6cm de largo, 4cm de ancho y 2,5cm de alto, que alcanzaba los ventrículos laterales y el tercer ventrículo, asociado a probable rotura de vasos del plexo coroideo. Los signos clínicos fueron inespecíficos, sugiriendo alteración en el sistema neurológico, sin embargo, debido a la rápida evolución, no fue posible realizar exámenes adicionales ni tratamiento. La necropsia fue fundamental para obtener un diagnóstico definitivo de ictus hemorrágico.O acidente vascular cerebral é incomum em pequenos animais, sendo classificado em isquêmico ou hemorrágico. Esta alteração cursa com sinais neurológicos inespecíficos e, em casos graves, rápida evolução para óbito, sendo de difícil diagnóstico ante mortem. Nosso objetivo é relatar o caso de um canino apresentando acidente vascular encefálico hemorrágico. Foi atendida uma fêmea, canina, com 2 anos de idade, apresentando perda de apetite, apatia, cegueira e ataxia durante as últimas 24 horas. Durante avaliação clínica foi constatado cegueira total bilateral, incontinência fecal e urinária, hipotonia muscular e estado semicomatoso, sendo que o animal evoluiu rapidamente para o óbito. Na necropsia, observou-se no encéfalo um coágulo, medindo 6cm de comprimento, 4cm de largura e 2,5cm de altura, atingindo ventrículos laterais e terceiro ventrículo, associado a provável ruptura de vasos do plexo coroide.
