Bundle of prevention of Post-Intensive Care Syndrome: construction of a technology in a hospital

Resumo: Objective: to describe the construction of a bundle for the prevention of Post-Intensive Care Syndrome in patients who survived hospitalization in the Intensive Care Unit. Methodology: This is a methodological, descriptive research with a quantitative approach. For the first moment, data collection (field research) was carried out through the analysis of medical records of patients admitted to the Intensive Care Center. The second moment was an integrative literature review in PubMed, LILACS and SciELO databases, which evaluated 7 studies that talk about the theme. Results: In total, 21 medical records of patients likely to have Post Intensive Care Syndrome were analyzed. The following results were obtained that promote the syndrome: preventive care of respiratory physiotherapy, pause in sedation and assessment of the possibility of extubation; short-term signs and symptoms; in the physical sphere, the most observed alteration was paresis; in the cognitive sphere, it was changes in sleep and wakefulness and, in the psychological sphere, it was anxiety. In view of the epidemiological analysis of the patients involved, the female sex was the most prevalent; the age group corresponded to 50-59 years; the most prevalent class of diagnoses, as a reason for hospitalization, was the postoperative period of general surgery and the length of stay in days that was equal to 11-15 days. All described, as steps of the prevention bundle, the “ABCDE” package. Conclusion: Finally, the bundle was designed to be applied in practice. This consists of the three spheres already mentioned and with care such as: bed exercises, sedation adjustment and pain assessment.

Metodología: Se trata de una investigación metodológica, descriptiva con enfoque cuantitativo. En un primer momento, la recolección de datos (investigación de campo) se realizó a través del análisis de las historias clínicas de los pacientes ingresados en el Centro de Cuidados Intensivos. El segundo momento fue una revisión integradora de literatura en las bases de datos PubMed, LILACS y SciELO, que evaluó 7 estudios que hablan sobre el tema. Resultados: En total se analizaron 21 historias clínicas de pacientes con probabilidad de presentar Síndrome Post Cuidados Intensivos. Se obtuvieron los siguientes resultados que favorecen el síndrome: atención preventiva de fisioterapia respiratoria, pausa en la sedación y valoración de la posibilidad de extubación; signos y síntomas a corto plazo; en el ámbito físico, la alteración más observada fue la paresia; en el ámbito cognitivo, los cambios en el sueño y la vigilia y, en el ámbito psicológico, la ansiedad. En vista del análisis epidemiológico de los pacientes involucrados, el sexo femenino fue el más prevalente; el grupo de edad correspondió a 50-59 años; la clase de diagnóstico más prevalente, como motivo de hospitalización, fue el postoperatorio de cirugía general y el tiempo de estancia en días que fue igual a 11-15 días. Todo descrito, como pasos del paquete de prevención, el paquete “ABCDE”. Conclusión: Finalmente, el paquete fue diseñado para ser aplicado en la práctica. Este consta de las tres esferas ya mencionadas y con cuidados como: ejercicios de cama, ajuste de la sedación y valoración del dolor.Objetivo: descrever a construção de um bundle para prevenção da Síndrome Pós-Cuidados Intensivos em pacientes sobreviventes de internação em Unidade Terapia Intensiva. Metodologia: Trata-se de uma pesquisa metodológica, descritiva, com abordagem quantitativa. Para o primeiro momento foi feita a coleta de dados (pesquisa de campo), realizada através da análise de prontuários de pacientes internados no Centro de Terapia Intensiva. O segundo momento frealizou-se uma revisão integrativa de literatura nas bases de dados PubMed, LILACS e SciELO, que avaliou 7 estudos que falam da temática. Resultados: No total, foram analisados 21 prontuários de pacientes propensos a apresentar Síndrome Pós Cuidados Intensivos. Obteve-se como resultados que propicima a síndrome: os cuidados preventivos de fisioterapia respiratória, pausa na sedação e avaliação da possibilidade de extubação; os sinais e sintomas de curto prazo; na esfera física, a alteração mais observada foi paresia; na esfera cognitiva foram alterações no sono e vigília e, na esfera psicológica, foi a ansiedade. Já diante da análise epidemiológica dos pacientes envolvidos, o sexo feminino foi o mais prevalente; a faixa etária correspondeu a 50-59 anos; a classe de diagnósticos mais prevalente, como motivo de internação, foi o pós-operatório de cirurgia geral e o tempo de permanência em dias que foi igual a 11-15 dias. Todos descreveram, como etapas do bundle de prevenção, o pacote “ABCDE”. Conclusão: Por fim, foi elaborado o bundle para ser aplicado na prática.
