Resumo: Ranula is defined as a pseudocyst originated from the leakage of mucinoid fluid of the sublingual gland, often associated with trauma or obstruction of the gland duct. Clinically, characterized by bluish and translucent color on the floor of the mouth and can be classified as an intraoral or plunging ranula. Diagnosis includes detailed clinical and imaging exams combined with histopathological findings. Therapeutic approaches can be applied to treat this lesion, such as marsupialization, excision of the ranula with or without the sublingual gland and excision of the ranula together the sublingual and submandibular gland. The aim of this paper was to report a case of plunging ranula, treated by excision followed by marsupialization of the cystic lesion, after one year of follow-up with recurrence. We conclude that a less invasive treatment may not present the best strategy for the treatment of plunging ranula, due to the greater chances of recurrence.

Clínicamente, se caracteriza por un color azulado y translúcido en el piso de la boca y puede clasificarse como una ránula intraoral o hundida. El diagnóstico incluye exámenes clínicos y por imágenes detallados combinados con hallazgos histopatológicos. Se pueden aplicar enfoques terapéuticos para tratar esta lesión, como marsupialización, escisión de la ránula con o sin la glándula sublingual y escisión de la ránula junto con la glándula sublingual y submandibular. El objetivo de este trabajo fue reportar un caso de ránula hundida, tratada mediante exéresis seguida de marsupialización de la lesión quística, luego de un año de seguimiento con recidiva. Concluimos que un tratamiento menos invasivo puede no presentar la mejor estrategia para el tratamiento de la ránula hundida, debido a las mayores posibilidades de recurrencia.Rânula é definida como um pseudocisto originada do extravasamento de líquido mucinoide da glândula sublingual, frequentemente associado a trauma ou obstrução do ducto da glândula. Clinicamente, caracterizada por uma coloração azulada, translúcida localizada no assoalho bucal e pode ser classificada como rânula intraoral ou mergulhante. O diagnóstico inclui exame clínico detalhado e exames de imagens combinados com achados histopatológicos. Abordagens terapêuticas podem ser aplicados para tratamento esta lesão, como marsupialização, exérese da rânula associada ou não a glândula sublingual e exérese da rânula juntamente a glândula sublingual e submandibular. O objetivo deste artigo foi relatar um caso de rânula mergulhante, tratada por exérese seguida de marsupialização da lesão cística, após um ano de acompanhamento com recidiva.
