Implementation of the group of chronically ill patients facing the covid-19 pandemic: experience report

Resumo: Objective: This research aims to describe the experiences of implementing an educational group for patients with hypertension and diabetes mellitus during the covid-19 pandemic period. Methodology: The study consists of an experience report of the researcher, through participant observation. Results: The monitoring of chronic patients through the educational group brought about a greater bonding of patients, improved adherence to treatment, improved health promotion actions by the team, greater accountability of the patient for their care and consequent improvement in the quality of life of the participants. Furthermore, the implementation of the educational group became an instrument for the follow-up of chronic patients by the Family Health Team. Conclusion: The identification of experiences in the organization and implementation of the educational group for chronically ill patients is of paramount importance for sharing actions that can help improve patient care in Primary Health Care.

Metodología: El estudio consiste en un relato de la experiencia de la investigación, a través de la observación participante. Resultados: El acompañamiento de los pacientes crónicos por medio del grupo educativo supone una mayor vinculación de los pacientes, la mejora de la adhesión a los tratamientos, el aprimoramiento de las acciones de promoción de la salud por parte del equipo, una mayor responsabilización del paciente por su cuidado y la consiguiente mejora de la calidad de vida de los participantes. Además, la implementación del grupo educativo se convirtió en un instrumento de acompañamiento de los pacientes crónicos por parte del Equipo de Salud de la Familia. Conclusión: La identificación de las experiencias de organización y aplicación del grupo educativo de pacientes crónicos es de suma importancia para el intercambio de acciones que puedan ayudar a la mejora de la asistencia a los pacientes en el ámbito de la Atención Primaria a la Salud.Objetivo: Esta pesquisa tem como objetivo principal descrever as vivências da implementação do grupo educativo de pacientes portadores de hipertensão e diabetes mellitus, em período de pandemia de covid-19. Metodologia: O estudo consiste em um relato de experiência da pesquisadora, através da observação participante. Resultados: O acompanhamento dos pacientes crônicos por meio do grupo educativo trouxe uma maior vinculação dos pacientes, melhoria da adesão aos tratamentos, aprimoramento das ações de promoção da saúde pela equipe, maior responsabilização do paciente pelo seu cuidado e consequente melhoria da qualidade de vida dos participantes. Ademais, a implementação do grupo educativo se tornou um instrumento de acompanhamento dos pacientes crônicos pela Equipe de Saúde da Família.
