TMJ alterations and their relationship with the appearance of dentofacial deformities: review of the literature

Resumo: | Alterations in the temporomandibular joint have varied etiologies, and may negatively affect both the functionality and the facial aesthetics of the individual. The mandibular condyle, one of the main components of the TMJ, is an adaptive growth site that influences the final conformity of the face, therefore, the relationship between TMJ pathologies and dentofacial deformities is conspicuous. Therefore, the objective of this study is to explain changes in the temporomandibular joint, especially in the condyle region, and relate them to the appearance of dentofacial deformities. The present work was carried out through a narrative literature review based on articles indexed in SciELO, Google Scholar, PubMed and the Virtual Health Library. Scientific articles, master's thesis and books, published in the period from 2010 to 2022, in full, written in Portuguese, English and Spanish, were included, and works published before 2010 and that were unrelated to the topic addressed were excluded. Thus, 31 works were selected for the preparation of this work, where the TMJ changes that are related to the appearance of dentofacial deformities addressed in the present work are: hyperplasia, hypoplasia and condylar aplasia, osteoma, osteochondroma and ankylosis of the temporomandibular joint. Therefore, it was possible to perceive that the physiological development of the TMJ, especially that of the condylar structure, without any alteration that could harm it, such as trauma and infections, is essential to avoid conversion into dentofacial deformities. |

Las alteraciones en la articulación temporomandibular tienen etiologías variadas y pueden afectar negativamente tanto la funcionalidad como la estética facial del individuo. El cóndilo mandibular, uno de los principales componentes de la ATM, es un sitio de crecimiento adaptativo que influye en la conformación final de la cara, por lo que es notoria la relación entre las patologías de la ATM y las deformidades dentofaciales. Por lo tanto, el objetivo de este estudio es explicar los cambios en la articulación temporomandibular, especialmente en la región del cóndilo, y relacionarlos con la aparición de deformidades dentofaciales. El presente trabajo se realizó a través de una revisión bibliográfica narrativa a partir de artículos indexados en SciELO, Google Scholar, PubMed y la Biblioteca Virtual en Salud. Se incluyeron artículos científicos, tesis de maestría y libros, publicados en el período de 2010 a 2022, en su totalidad, escritos en portugués, inglés y español, y se excluyeron los trabajos publicados antes de 2010 y que no tuvieran relación con el tema abordado. Así, para la elaboración de este trabajo se seleccionaron 31 trabajos, donde las alteraciones de la ATM que se relacionan con la aparición de deformidades dentofaciales abordadas en el presente trabajo son: hiperplasia, hipoplasia y aplasia condilar, osteoma, osteocondroma y anquilosis de la articulación temporomandibular. Por lo tanto, fue posible percibir que el desarrollo fisiológico de la ATM, especialmente de la estructura condilar, sin ninguna alteración que pueda dañarla, como traumatismos e infecciones, es fundamental para evitar la conversión en deformidades dentofaciales. Alterações na articulação temporomandibular possuem variadas etiologias, podendo repercutir de forma negativa tanto na funcionalidade quanto na estética facial do indivíduo. O côndilo mandibular, um dos principais componentes da ATM, é um local de crescimento adaptativo que influencia na conformidade final da face, por isso, é conspícuo a relação entre as patologias da ATM e as deformidades dentofaciais. Portanto, o objetivo desse estudo é explanar alterações da articulação temporomandibular, sobretudo em região de côndilo, e relacioná-las com o surgimento de deformidades dentofaciais. O presente trabalho foi realizado por meio de uma revisão de literatura narrativa com base em artigos indexados no SciELO, Google Acadêmico, PubMed e Biblioteca Virtual de Saúde. Foram incluídos artigos científicos, dissertação de mestrado e livros, publicados no período de 2010 a 2022, de forma completa, escritos em português, inglês e espanhol, e excluídos trabalhos publicados antes de 2010 e que não tinham relação com o tema abordado. Assim, foram selecionadas 31 obras para a confecção deste trabalho, onde as alterações da ATM que possuem relação com o surgimento de deformidades dentofaciais abordadas no presente trabalho, são: hiperplasia, hipoplasia e aplasia condilar, osteoma, osteocondroma e anquilose da articulação temporomandibular. Portanto foi possível perceber que o desenvolvimento fisiológico da ATM, especialmente o da estrutura condilar, sem qualquer alteração que possa prejudica-la, como traumas e infecções, é essencial para evitar a conversão em deformidades dentofaciais.
