Adverse events related health care in Brazil: An ecologic study between 2014-2018

Resumo: Objectives: To identify the types of incidents most notified to the National Health Surveillance Agency through the computerized system, since the implementation of patient safety centers in the country. Method: Ecological study based on the data obtained from the Patient Safety and Quality in Health Services Information Bulletins related to health care issued by ANVISA and the National Registry of Health Facilities (CNES) from 2014 to 2018. Results: The data were subjected to the Kolmogorov-smirnovi normality test, and a non-parametric approach was chosen (Sperman, Wilcox, Kruskal-Wallis) with a significance level of 5%. The results showed that both in the biennia 2015-2016 and 2017-2018 the most prevalent types of incidents were: failure during assistance (27.8% /22.2%); pressure injury (17% / 18.6%); fall (11.1% / 11%); failure to identify the patient (6.3% / 8%); loss or obstruction of tube (7.5% / 6.5%) and phlebitis (5.4% / 4.9%). Conclusion: Despite the advances achieved, much remains to be done to establish safety culture in health institutions and to report these adverse events.

Método: Estudio ecológico con base en datos obtenidos de los Boletines de Información sobre Seguridad y Calidad del Paciente en los Servicios de Salud relacionados con la Atención de Salud emitidos por ANVISA y el Registro Nacional de Establecimientos de Salud (CNES) de 2014 a 2018. Resultados: Los datos fueron remitidos a la Prueba de normalidad de Kolmogorov-smirnovi, eligiéndose un abordaje no paramétrico (Sperman, Wilcox, Kruskal-Wallis) con un nivel de significancia del 5%. Los resultados mostraron que en los bienios 2015-2016 y 2017-2018 los tipos de incidentes más prevalentes fueron: fallas durante la asistencia (27,8% / 22,2%); lesión por presión (17% / 18,6%); caída (11,1% / 11%); falta de identificación del paciente (6,3% / 8%); pérdida u obstrucción de la trompa (7,5% / 6,5%) y flebitis (5,4% / 4,9%). Conclusión: A diferencia de otros países donde las fallas más prevalentes se relacionaron con infecciones, medicamentos y procedimientos quirúrgicos, los eventos adversos en Brasil están más relacionados con la atención de enfermería. A pesar de los avances logrados, queda mucho por hacer para establecer una cultura de seguridad del paciente en las instituciones de salud y superar las barreras existentes en cuanto a la notificación de estos eventos, siendo el sistema de notificación de incidentes una estrategia importante en la identificación de problemas en la zona. seguridad del paciente, fomentando el aprendizaje y mejorando los procesos asistenciales.Objetivos: Identificar os tipos de incidentes mais notificados no Brasil desde a implantação dos núcleos de segurança do paciente. Método: Estudo epidemiológico ecológico com base nos dados obtidos nos Boletins Informativos de Segurança do Paciente e Qualidade em Serviços de Saúde relacionados a assistência à Saúde emitidos pela ANVISA e do Cadastro Nacional de Estabelecimentos de Saúde (CNES) dos anos de 2014 a 2018.  Resultados: Os dados foram submetidos a teste de normalidade de Kolmogorov-smirnovi, sendo escolhida uma abordagem não-paramétrica (Sperman, Wilcox, Kruskal-Wallis) com nível de significância de 5%. Os resultados evidenciaram que nos biênios 2015-2016 e 2017-2018 os tipos de incidentes mais prevalentes foram: falha durante a assistência (27,8%/22,2%); lesão por pressão (17%/18,6%); queda (11,1%/11%); falha na identificação do paciente (6,3%/8%); perda ou obstrução de sonda (7,5%/ 6,5%) e flebite (5,4% /4,9%). Conclusão: Diferente de outros países onde as falhas mais prevalentes estiveram relacionadas a infecção, medicamentos e procedimentos cirúrgicos, os eventos adversos no Brasil estão mais relacionados aos cuidados de enfermagem.
