Hospital discharge planning as a continuity care strategy for primary care

Resumo: Objective: to analyze hospital discharge planning as a strategy of continuity of care for Primary Health Care. Method: qualitative study, carried out in a university hospital in the southern region of the country. Online questionnaire applied with Nurses and interview with the Director of Nursing, between October and November 2019 and Content Analysis to analyze the data collected. Results: Nurses conducted interviews and clinical assessment of patients during hospitalization and for planning discharge from hospital. In preparing for hospital discharge, several professionals were involved, mainly doctors and nurses, however, there was no counter-referral protocol, with the summary of medical and nursing discharges for the continuity of care for Primary Health Care. The Nurses guided the family about home care, assessed socioeconomic conditions and the home environment. Conclusion: the use of protocols at hospital discharge allows nurses to follow planned care and promote continuity of care for Primary Care.

Cuestionario conectado aplicado con enfermeros y entrevista con la directora de enfermería, entre octubre y noviembre de 2019, y análisis de contenido para analizar los datos recopilados. Resultados: Los enfermeros realizaron entrevistas y evaluaciones clínicas de pacientes durante la hospitalización y para planificar el alta hospitalaria. Al prepararse para el alta hospitalaria, se contó con la participación de varios profesionales, principalmente médicos y enfermeros, sin embargo, no hubo un protocolo de contrarreferência, con el resumen de los egresos médicos y de enfermería para la continuidad de la atención de la Atención Primaria de Salud; los enfermeros orientaron a la familia sobre el cuidado en el hogar, evaluaron las condiciones socioeconómicas y el entorno del hogar. Conclusión: el uso de protocolos en el alta hospitalaria permite a los enfermeros seguir la atención planificada y promover la continuidad de la atención primaria.Objetivo: analisar o planejamento da alta hospitalar como estratégia de continuidade do cuidado para Atenção Primária à Saúde. Método: estudo qualitativo, realizado em hospital universitário na região sul do país. Aplicado questionário online com os enfermeiros e entrevista com a Diretora de Enfermagem, entre outubro e novembro de 2019 e a Análise de Conteúdo para analisar os dados coletados. Resultados: os enfermeiros realizavam entrevista e avaliação clínica dos pacientes na internação e para o planejamento da alta hospitalar. No preparo da alta hospitalar, houve envolvimento de vários profissionais, principalmente dos médicos e enfermeiros, no entanto, não havia protocolo de contrarreferência, com o resumo das altas médicas e da enfermagem para a continuidade da assistência ao paciente para a Atenção Primária à Saúde. Os enfermeiros orientavam a família sobre os cuidados no domicílio, avaliavam as condições socioeconômicas e o ambiente domiciliar.
