Evolutions in the surgical treatment of breast cancer: a literature review

Resumo: | Introduction: Because it is a very common neoplasm worldwide, the treatment of breast cancer is always updated, aiming to increase the quality of life and aesthetic satisfaction of patients. Objective: To expose the developments in the surgical treatment of breast cancer, according to the world literature. Methodology: A literature review was carried out with the following thematic question: what are the current innovations in surgical treatments for breast cancer? The descriptors "breast cancer AND surgery" were searched in the following databases: SciELO, PubMED and LILACS. Inclusion criteria were articles published in the last 3 years, in Portuguese, English or Spanish, and that answered the guiding question. Commentaries and editorials, paid or incomplete articles, duplicates and studies that did not fit the theme were excluded. Results: a total of 5963 articles were obtained, reduced to a final sample of 12 articles after selection based on the inclusion and exclusion criteria. Oncoplastic surgery proved to be the most current practice for the treatment of breast cancer, along with sentinel lymph node biopsy and intraoperative radiation. The main benefits cited were improvement in the quality of life and in the patient's self-esteem. Final Considerations: oncoplasty should be included in the oncologic surgeon's training, so that he can offer the best treatment for his patients. In addition, methods for the diagnosis of early breast cancer should be encouraged and studied, so that the treatment and prognosis are favored. |

Objetivo: Exponer los avances en el tratamiento quirúrgico del cáncer de mama, según la literatura mundial. Metodología: Se realizó una revisión bibliográfica con la siguiente pregunta-tema: ¿cuáles son las innovaciones en los tratamientos quirúrgicos del cáncer de mama en la actualidad? Para ello, se buscaron los descriptores "breast cancer AND surgery" en las siguientes bases de datos: SciELO, PubMED y LILACS. Los criterios de inclusión fueron artículos publicados en los últimos tres años, en portugués, inglés o español, que respondieran a la pregunta guía. Se excluyeron los comentarios y editoriales, los artículos pagados o incompletos, los duplicados y los estudios que no se ajustaban al tema. Resultados: se obtuvo un total de 5963 artículos, reducidos a una muestra final de 12 artículos tras la selección basada en los criterios de inclusión y exclusión. La cirugía oncoplástica resultó ser la práctica más actual para el tratamiento del cáncer de mama, junto con la biopsia del ganglio linfático centinela y la radiación intraoperatoria. Los principales beneficios citados fueron la mejora de la calidad de vida y de la autoestima del paciente. Consideraciones finales: la oncoplastia debería incluirse en la formación de los cirujanos oncológicos, para que puedan ofrecer el mejor tratamiento a sus pacientes. Además, deben fomentarse y estudiarse los métodos para el diagnóstico del cáncer de mama precoz, de modo que se favorezca el tratamiento y el pronóstico.Introdução: Por ser uma neoplasia muito comum em todo o mundo, a terapêutica do câncer de mama sempre é atualizada, visando aumentar a qualidade de vida e a satisfação estética dos pacientes. Objetivo: expor as evoluções no tratamento cirúrgico da neoplasia mamária, de acordo com a literatura mundial. Metodologia: Realizou-se uma revisão de literatura com a seguinte pergunta-tema: quais as inovações dos tratamentos cirúrgicos do câncer de mama na atualidade? Para isso, buscou-se pelos descritores “breast câncer AND surgery” nas seguintes bases de dados: SciELO, PubMED e LILACS. Os critérios de inclusão foram artigos publicados nos últimos 3 anos, em idioma português, inglês ou espanhol, e que respondessem à questão norteadora. Foram excluídos comentários e editoriais, artigos pagos ou incompletos, duplicatas e estudos que fugissem à temática. Resultados: foram obtidos 5963 artigos no total, reduzidos a uma amostra final de 12 artigos após seleção com base nos critérios de inclusão e de exclusão. A cirurgia oncoplástica se mostrou a prática mais atual para o tratamento do câncer de mama, juntamente com a biópsia de linfonodo sentinela e a radiação intraoperatória. Os principais benefícios citados foram a melhoria na qualidade de vida e na autoestima da paciente. Considerações Finais: a oncoplastia deve ser inserida na formação do cirurgião oncológico, de modo que este possa ofertar o melhor tratamento para seus pacientes. Além disso, métodos para o diagnóstico do câncer de mama precoce devem ser incentivados e estudados, para que o tratamento e o prognóstico sejam favorecidos.
