Surgical treatment of impacted canine in the mandible basis: a case report

Resumo: | The occurrence of impacted lower canines is not commonly observed in dental clinical practice. Several treatment options might be proposed for these cases. This paper aims to report the surgical treatment of an included canine in the base of the mandible. This is a 13-year-old male patient who sought orthodontic treatment aiming dental alignment due to the presence of non-exfoliated deciduous canine. With the aid of panoramic radiography and computed tomography, it was observed the horizontal position of the right lower canine included, as well the advanced stage of root formation of the tooth in question. There was also a radiolucent image involving the crown of the same tooth, suggesting cystic lesion. These conditions showed the impossibility of orthodontic treatment for repositioning the permanent canine in the lower arch. Thus, the planning and surgical treatment were performed with great precision. No trans and postoperative complications were observed. From the data obtained in the literature and by analyzing the case in question clinically and radiographically, the most viable treatment in this case was the surgical removal of the canine. |

Se proponen varias modalidades de tratamiento para estos casos. Este trabajo tiene como objetivo informar el tratamiento quirúrgico de un canino incluido en la región de la base de la mandíbula. Este es un paciente masculino de 13 años que buscó tratamiento de ortodoncia con el objetivo de alinear los dientes debido a la presencia de un canino caducifolio no exfoliado. Con la ayuda de imágenes de radiografía panorámica y tomografía computarizada, se observó la posición horizontal del canino inferior derecho incluido, una etapa avanzada de formación de raíces del diente en cuestión, así como una imagen radiolúcida que involucra la corona del mismo diente, lo que sugiere una lesión quística. evidenciando la imposibilidad de realizar un tratamiento de ortodoncia para reposicionar el canino permanente en el arco inferior. Así, la planificación y el tratamiento quirúrgico se llevaron a cabo con gran precisión. No se observaron complicaciones trans y postoperatorias. A partir de los datos obtenidos en la literatura y al analizar el caso en cuestión clínica y radiográficamente, el tratamiento más viable fue la extirpación quirúrgica.A ocorrência de caninos inferiores impactados não é comumente observada na prática clínica odontológica. Diversas modalidades de tratamento são propostas para estes casos. Este trabalho tem por objetivo relatar a realização de tratamento cirúrgico de um canino incluso na região da base da mandíbula. Trata-se de um paciente do gênero masculino, 13 anos, que procurou tratamento ortodôntico objetivando alinhamento dental devido a presença de canino decíduo não esfoliado. Com o auxílio de imagens de radiografia panorâmica e tomografia computadorizada, foi observado a posição horizontal do canino inferior direito incluso, estágio avançado de formação da raiz do dente em questão, bem como imagem radiolúcida envolvendo a coroa do mesmo dente, sugestivo de lesão cística, evidenciando a inviabilidade de realização de tratamento ortodôntico para reposicionamento do canino permanente na arcada inferior. Com isso, o planejamento e tratamento cirúrgico foram realizados com grande precisão. Não foi observada a ocorrência de complicações trans e pós-operatórias.
