Protocol for surgical treatment of patients using anticoagulants and antiplatelet agents

Resumo: | It know that currently in the routine of dental offices the frequency of patients who use antithrombotic medications such as anticoagulants and antiplatelet agents is quite relevant, these drugs act to decrease blood coagulability, preventing thromboembolic problems from occurring. And with that it brings the diversity of thoughts in relation to performing dental surgical procedures. With the evolution of dentistry, it is possible to observe the diversity of existing protocols for the care of these patients, ranging from the change of heparin, reduction or pause of antithrombotic medication, to its maintenance, emphasizing the use of local hemostatic techniques. In view of the use of antithrombotic therapy by these individuals who started to make constant dental consultations, the purpose of this study was to analyze the most appropriate way to perform the care of these patients who are subjected to oral surgical procedures. The INR test is a calculation performed between the prothrombin time and the average of the normal coagulation interval, where it demonstrates that it is reliable to perform dental surgery if the INR is between 2 and 3.5. If it is higher than 5, it is not recommended to perform any procedure. Some authors claim that it is reliable to perform dental surgeries without major bleeding and in a safe manner, as long as the International Standardized List (INR) is included in the therapeutic levels. In this perspective, it was possible to analyze that maintenance on anticoagulant therapy, associated with the use of local hemostasis methods, represents the most appropriate protocol for surgical cases at an outpatient level. |

Se sabe que em laactualidad em la rutina de losconsultorios odontológicos lafrecuencia de pacientes que utilizan medicamentos antitrombóticos, como anticoagulantes y antiagregantesplaquetarios, es considerable. Estos medicamentos actúan para diminuir lacoagulabilidad de la sangre, evitando que ocurran problemas tromboembólicos. Com laevolución de losestudios sobre este tema, es posible observar que existe una diversidad de protocolos propuestos em la literatura que apuntan a laatención segura de estos pacientes. Estos protocolos van desde el intercambio de fármacos, reducción o pausa de lamedicación antitrombótica, hasta sumantenimiento, destacando el uso de técnicas hemostáticas locales. Ante el uso de terapia antitrombótica por estosindividuos que comenzaron a realizar consultas dentales constantes, el propósito de este estúdio fueanalizar, mediante una revisiónde la literatura, la forma más adecuada de realizar el cuidado de estos pacientes que son sometidos a procedimientos. Procedimentos quirúrgicosorales. La prueba de INR esun cálculo que se realiza entre eltiempo de protrombina y la media del intervalo de coagulación normal, donde de muestra que es confiable para realizar cirugía dental si el INR se encuentra entre 2 y 3,5. Si es superior a 5, no se recomienda realizar ningúnprocedimiento. Algunos autores afirman que esfiable realizar cirugíasdentalessin grandes hemorragias y de forma segura, siempre que el INR se mantengaen niveles seguros. En esta perspectiva, se pudoanalizar que elmantenimiento de la terapia anticoagulante, asociado al uso de métodos de hemostasia local, representa el protocolo más adecuado para los casos quirúrgicos a nível ambulatorio.Sabe-se que atualmente na rotina dos consultórios odontológicos a frequência de pacientes que fazem o uso de anticoagulantes e antiagregantes plaquetários, é considerável. Esses medicamentos atuam na diminuição da coagulabilidade do sangue, evitando problemas tromboembólicos. Com a evolução dos estudos a cerca deste tema, é possível observar que há uma diversidade de protocolos propostos na literatura que visam o atendimento seguro desses pacientes. Esses protocolos vão desde a troca dos medicamentos, diminuição ou pausa da medicação antitrombótica, até sua manutenção, ressaltando o emprego de técnicas hemostáticas locais. Tendo em vista a utilização de terapia antitrombótica por esses indivíduos que passaram a fazer consultas odontológicas constantes, o propósito deste estudo foi analisar, por meio de uma revisão da literatura, através da pesquisa nas bases de dados Scielo, Bireme e Pubmed, a forma mais adequada para realizar o atendimento desses pacientes que são sujeitos à realização de cirurgias orais. O teste de INR é um cálculo realizado entre o tempo de protrombina e a média do intervalo normal da coagulação, onde demonstra que é confiável realizar cirurgias odontológicas se o INR estiver entre 2 e 3,5. Caso esteja superior a 5, não é recomendado executar nenhum procedimento. Alguns autores afirmam que é confiável realizar cirurgias odontológicas sem grandes sangramentos e de forma segura, desde que o INR esteja em níveis seguros.
