Pilates method in diabetes control in pregnant women: a systematic review

Resumo: Pregnancy is a period of many changes in a woman's body designed to promote the growth of the fetus. Gestational diabetes mellitus (GDM) is defined as carbohydrate intolerance that occurs for the first time during pregnancy. Physical exercise has minimal benefits and risks for pregnant women, and is important for the health of women with gestational diabetes mellitus. The Pilates method is a system of exercises based on the philosophy and principles of the body, mind and spirit. This study aimed to show if the practice of Pilates is really effective when compared to the effects of standard physical exercise during pregnancy in women with diabetes, so that this practice will contribute to the control of blood glucose levels. Searches were carried out in the following databases: PEDro (Physiotherapy Evidence Database), PubMed (National Library of Medicine) and VHL (Virtual Health Library). In the results, only 04 randomized clinical trials out of 116 were relevant and that met the eligibility criteria according to the methodological quality assessment of the PEDro Scale. In the discursive analysis, the articles exposed in this study were analyzed according to the gestational age of the participants, standard physical exercise interventions regarding the reduction of diabetes mellitus in pregnant women. However, there were no statistically significant differences between the intervention groups, so the evidence was insufficient to suggest that physical exercise during pregnancy can be effective in reducing the risk of gestational diabetes mellitus and in reducing blood glucose levels.

La diabetes mellitus gestacional (DMG) se define como la intolerancia a los carbohidratos que ocurre por primera vez durante el embarazo. El ejercicio físico tiene beneficios y riesgos mínimos para las mujeres embarazadas y es importante para la salud de las mujeres con diabetes mellitus gestacional. El método Pilates es un sistema de ejercicios basado en la filosofía y principios del cuerpo, la mente y el espíritu. Este estudio tuvo como objetivo mostrar si la práctica de Pilates es realmente efectiva en comparación con los efectos del ejercicio físico estándar durante el embarazo en mujeres con diabetes, de modo que esta práctica contribuya al control de los niveles de glucosa en sangre. Las búsquedas se realizaron en las siguientes bases de datos: PEDro (Physiotherapy Evidence Database), PubMed (National Library of Medicine) y BVS (Virtual Health Library). En los resultados, solo 04 ensayos clínicos aleatorizados de 116 fueron relevantes y cumplieron con los criterios de elegibilidad según la evaluación de la calidad metodológica de la Escala PEDro. En el análisis discursivo, los artículos expuestos en este estudio fueron analizados según la edad gestacional de las participantes, intervenciones estándar de ejercicio físico en relación a la reducción de la diabetes mellitus en gestantes. Sin embargo, no hubo diferencias estadísticamente significativas entre los grupos de intervención, por lo que la evidencia fue insuficiente para sugerir que el ejercicio físico durante el embarazo puede ser efectivo para reducir el riesgo de diabetes mellitus gestacional y reducir los niveles de glucosa en sangre.A gestação é um período de muitas mudanças no corpo da mulher destinadas a promover o crescimento do feto. O exercício físico tem benefícios e riscos mínimos para mulheres grávidas, e é importante para a saúde de mulheres com diabetes mellitus gestacional. O método pilates é um sistema de exercícios com base na filosofia e princípios do corpo, mente e espirito. Este estudo teve como objetivo mostrar se a prática do pilates é realmente efetiva quando comparada aos efeitos do exercício físico padrão durante a gestação em mulheres com diabetes, de modo que essa prática irá contribuir para o controle dos níveis de glicemia no sangue. Foram realizadas as buscas nas bases: PEDro (Physiotherapy Evidence Database), PubMed (National Library of Medicine) e BVS (Biblioteca Virtual de Saúde). Nos resultados apenas 04 ensaios clínicos randomizados de 116 foram relevantes e que cumpriram os critérios de elegibilidade de acordo com avaliação da qualidade de metodológica da Escala PEDro. Na análise discursiva os artigos expostos neste estudo, foram analisados de acordo com idade gestacional das participantes, intervenções de exercícios físicos padrão quanto a redução da diabetes mellitus em gestantes.
