Adherence to tuberculosis treatment in Primary Health Care: favorable and unfavorable factors for this process

Summary: Tuberculosis is an infectious disease of airborne transmission, caused by the Mycobacterium tuberculosis complex, which is configured as a serious public health problem, among other factors, by the abandonment of treatment by users. The present study aimed to identify the main concepts and factors associated with non-adherence to tuberculosis care in Primary Health Care. This is a narrative review of literature, conducted on the following platforms: PubMed, LILACS, SciELO and the CAPES Periodical Portal, where 1634 studies were found, 18 of which met the inclusion criteria to be read and analyzed in full. The main findings were: deficiencies in the organization of the health system at the level of articulation between levels of health care and fragility in the completion of notification instruments, which make the supply of services ineffective for the control of tuberculosis and, coupled with socially vulnerable patients, potentiate the abandonment of tuberculosis treatment. Finally, it was possible to achieve the general objective of this study, identifying the main concepts and factors associated with non-adherence to tuberculosis care, which is a disease with a high possibility of cure, and at the same time a high percentage of deaths in Brazil and worldwide.

El objetivo de este estudio es identificar los principales conceptos y factores asociados a la no adherencia a la atención de la tuberculosis en Atención Primaria. Se trata de una revisión narrativa de la literatura, realizada en las plataformas: PubMed, LILACS, SciELO y el Portal de Periódicos de CAPES, donde se encontraron 1634 estudios, de los cuales 18 cumplieron los criterios de inclusión para ser leídos y analizados en su totalidad. Los principales hallazgos fueron: deficiencias en la organización del sistema de salud a nivel de articulación entre los niveles de atención y fragilidad en el diligenciamiento de los instrumentos de notificación, lo que hace que la oferta del servicio sea ineficaz para el control de la tuberculosis y, combinado con los pacientes socialmente vulnerables, potencian el abandono del tratamiento de la tuberculosis. Finalmente, fue posible alcanzar el objetivo general de este estudio, identificando los principales conceptos y factores asociados a la no adherencia al cuidado de la tuberculosis, que es una enfermedad con alta posibilidad de curación, y al mismo tiempo alto porcentaje de muertes en Brasil y en el mundo.A tuberculose é uma doença infectocontagiosa de transmissão aérea, causada pelo complexo Mycobacterium tuberculosis, que se configura como um grave problema de saúde pública, dentre outros fatores, pelo abandono de tratamento pelos usuários. O presente estudo teve como objetivo identificar quais os principais conceitos e fatores associados à não adesão aos cuidados da tuberculose na Atenção Primária à Saúde. Trata-se de uma revisão narrativa da literatura, realizada nas plataformas: PubMed, LILACS, SciELO e no Portal de Periódicos CAPES, onde foram encontrados 1634 estudos, dos quais 18 comtemplaram os critérios de inclusão para serem lidos e analisados na íntegra. Os principais achados foram: deficiências na organização do sistema de saúde a nível da articulação entre os níveis de atenção à saúde e fragilidade no preenchimento de instrumentos de notificação, que tornam a oferta do serviço ineficaz para o controle da tuberculose e, aliados a pacientes vulneráveis socialmente, potencializam o abandono do tratamento da tuberculose.
