Management and prognosis of gastroschisis in a public maternity hospital in the state of Sergipe

Resumo: | Gastroschisis is the most common congenital anomaly of the abdominal wall, defined as a defect in its closure, resulting in evisceration of intra-abdominal structures. It often appears isolated, without association with other anomalies. In different regions of Brazil and the world, there has been an increase in the incidence of this pathology - currently between 2 and 5 per 10,000 live births - configuring a public health problem to be equated. The present study aimed to analyze the profile of births and deaths due to gastroschisis, to identify the causes of in-hospital mortality and the occurrences of postoperative complications of newborns with gastroschisis, in a public maternity hospital in the state of Sergipe. This is a prospective observational cohort study, aligned with the multicenter study 'PAEDSURG' in which all Brazilian hospitals would be eligible to participate. Data were collected between 11/15/2021 and 11/15/2022, from all consecutively eligible cases, under the supervision of the pediatric surgeon coordinating the research site, Drª Marcia Montalvão. Five cases of newborns with gastroschisis were described, all born by cesarean section, prematurity was seen in 4 cases (median GA 35 weeks), with low birth weight observed in 75% of cases (median 2180g). All mothers were primiparous, all had prenatal care and underwent an ultrasound examination, showing gastroschisis in 4 NBs, 3 mothers had low maternal age (median of 18 years), none with higher education. The mortality rate was low (of the 5 NBs, only 1 died) and socioeconomic factors were associated with the occurrence of gastroschisis. |

A menudo aparece aislado, sin asociación con otras anomalías. En diferentes regiones de Brasil y del mundo, ha habido un aumento en la incidencia de esta patología - actualmente entre 2 y 5 por 10.000 nacidos vivos - configurando un problema de salud pública a equiparar. El presente estudio tuvo como objetivo analizar el perfil de nacimientos y muertes por gastrosquisis, identificar las causas de mortalidad intrahospitalaria y la ocurrencia de complicaciones postoperatorias de recién nacidos con gastrosquisis, en una maternidad pública del estado de Sergipe. Este es un estudio de cohorte observacional prospectivo, alineado con el estudio multicéntrico 'PAEDSURG' en el que todos los hospitales brasileños serían elegibles para participar. Los datos fueron recolectados entre el 15/11/2021 y el 15/11/2022, de todos los casos elegibles consecutivamente, bajo la supervisión de la cirujana pediátrica coordinadora del sitio de investigación, Dra. Marcia Montalvão. Se describieron cinco casos de recién nacidos con gastrosquisis, todos nacidos por cesárea, se observó prematuridad en 4 casos (mediana de EG 35 semanas), observándose bajo peso al nacer en el 75% de los casos (mediana 2180g). Todas las madres eran primíparas, todas tenían control prenatal y se les realizó un examen de ultrasonido, mostrando gastrosquisis en 4 RN, 3 madres tenían baja edad materna (mediana de 18 años), ninguna con educación superior. La tasa de mortalidad fue baja (de los 5 RN, solo 1 murió) y los factores socioeconómicos se asociaron con la aparición de gastrosquisis.A gastrosquise é a anomalia congênita mais comum da parede abdominal, definida como um defeito do seu fechamento, resultando em evisceração de estruturas intra-abdominais. Aparece, frequentemente, isolada, sem associação a outras anomalias. Em diferentes regiões do Brasil e do mundo, tem ocorrido um aumento de incidência dessa patologia - atualmente entre 2 a 5 por 10.000 nascidos vivos - configurando-se um problema de saúde pública a ser equacionado. O presente estudo objetivou analisar o perfil dos nascimentos e óbitos por gastrosquise, identificar as causas de mortalidade intra-hospitalares e as ocorrências de complicações pós-operatórias de RN com gastrosquise, em uma maternidade pública no estado de Sergipe. Trata-se de um estudo prospectivo observacional de coorte, alinhado ao estudo multicêntrico ‘PAEDSURG’ em que todos os hospitais brasileiros seriam elegíveis para participar. Os dados foram coletados entre 15/11/2021 e 15/11/2022, de todos os casos consecutivamente elegíveis, sob a supervisão da cirurgiã pediátrica coordenadora local da pesquisa, Drª Marcia Montalvão. Foram descritos 5 casos de RN com gastrosquise, todos nascido por cesárea, a prematuridade foi vista em 4 casos (mediana de IG 35 semanas), sendo o peso baixo ao nascimento observado em 75% dos casos (mediana de 2180g). Todas as mães eram primigestas, todas fizeram pré-natal e realizaram exame ultrassonográfico, evidenciando gastrosquise em 4 RN, 3 mães tinham baixa idade materna (mediana de 18 anos), nenhuma com nível superior.
