Recommendation and safety of vaccinacion in patients with rheumatoid arthritis using immunosuppressive treatment

Resumo: Introduction: Rheumatoid arthritis (RA) is an inflammatory autoimmune disease often treated using immunosuppressive drugs that are directly related to the occurrence of opportunistic manifestations. Therefore, vaccination can be a means of protection, but its safety and recommendation must be evaluated. Objective: To identify the safety and recommendation of vaccination in patients treating RA. Method: Integrative literature review, developed from 11 articles obtained from the PubMed® database, and 3 articles obtained from the BVS library in which the same descriptors were used in Health Sciences, totaling 14 analyzed works, a time frame was defined, covering articles from the last 5 years. Results: According to the findings, the main recommended and safe vaccines are Pneumococcal, Influenza, Hepatitis B, Human Papilloma Virus (HPV) and Herpes Zoster. However, attention must be paid to the interaction between vaccine-drug, as this relationship can cause damage to the performance of the vaccine or even harm to the patient. Conclusion: Based on the analyzed studies, it is concluded that vaccination is generally recommended and safe for individuals with rheumatoid arthritis undergoing treatment with immunosuppressants.

Por tanto, la vacunación puede ser un medio de protección, pero se debe evaluar su seguridad y recomendación. Objetivo: Identificar la seguridad y recomendación de vacunación en pacientes en tratamiento con AR. Método: Revisión bibliográfica integrativa, desarrollada a partir de 11 artículos obtenidos de la base de datos PubMed®, y 3 artículos obtenidos de la biblioteca BVS en los que se utilizaron los mismos descriptores en Ciencias de la Salud, totalizando 14 trabajos analizados, se definió un marco temporal, contemplando artículos de los últimos 5 años. Resultados: Según los hallazgos, las principales vacunas recomendadas y seguras son el neumococo, la influenza, la hepatitis B, el virus del papiloma humano (VPH) y el herpes zóster. Sin embargo, se debe prestar atención a la interacción entre la vacuna y el fármaco, ya que esta relación puede dañar el rendimiento de la vacuna o incluso dañar al paciente. Conclusión: Con base en los estudios analizados, se concluye que la vacunación es generalmente recomendada y segura para personas con artritis reumatoide en tratamiento con inmunosupresores.Introdução: A artrite reumatoide (AR) é uma doença autoimune inflamatória frequentemente tratada se utilizando drogas imunossupressoras que estão diretamente relacionadas a ocorrência de manifestações oportunistas. Logo, a vacinação pode ser um meio de proteção, porém deve ser avaliada sua segurança e recomendação.  Objetivo: Identificar a segurança e a recomendação da vacinação em pacientes tratando AR. Método: Revisão integrativa da literatura, desenvolvida a partir de 11 artigos obtidos da base de dados PubMed®, e 3 artigos obtidos na biblioteca BVS em que  foram utilizados mesmos descritores em Ciências da Saúde, totalizando 14 trabalhos analisados, foi definido um recorte temporal contemplando artigos dos últimos 5 anos. Resultados: Conforme os achados, as principais vacinas recomendadas e seguras são a Pneumocócica, Influenza, Hepatite B, Papiloma Vírus Humano (HPV) e Herpes Zoster. No entanto, deve-se se atentar com a interação entre vacina-medicamento, pois essa relação pode causar prejuízos na performance da vacina ou mesmo prejuízo ao paciente.
