Endodontic surgery in cases of persistent apical periodontitis: case report with 6-month follow-up

Resumo: The etiological factors of pulp alterations, mainly those of biological origin, can trigger inflammatory reactions in the periradicular tissues. Despite technological advances in endodontic treatments, increasing the predictability of success, we still face cases of persistent infection. When well indicated and performed correctly, with satisfactory success rates, endodontic surgery emerges as an option for a complementary procedure to endodontic disinfection. The present study aims to report a clinical case using the concept of endodontic microsurgery in a case of persistent apical periodontitis. A 39-year-old female patient, without systemic alterations, sought the dental clinic of the Endodontics Specialization course at Faculdade São Leopoldo Mandic, Belo Horizonte Unit, complaining of painful symptoms in the region of teeth 14 and 15. From the previous history of the tooth, it was reported by the patient that she underwent endodontic retreatment at the same institution about 5 months ago due to a previous treatment that was not successful. During the clinical examination, it was possible to observe the presence of a fistula in the vestibular region between the aforementioned teeth. After clinical and radiographic evaluation, it was decided to perform endodontic microsurgery, given the previously performed retreatment. The case report was approved by the Research Ethics Committee and is registered under CAAE number: 22295119.8.0000.5374 and opinion number: 3.651.194.

A pesar de los avances tecnológicos en los tratamientos de endodoncia, que aumentan la previsibilidad del éxito, todavía nos enfrentamos a casos de infección persistente. Cuando está bien indicada y realizada correctamente, con tasas de éxito satisfactorias, la cirugía endodóntica surge como una opción de procedimiento complementario a la desinfección endodóntica. El presente estudio tiene como objetivo reportar un caso clínico utilizando el concepto de microcirugía endodóntica en un caso de periodontitis apical persistente. Una paciente de 39 años, del sexo femenino, sin alteraciones sistémicas, buscó la clínica odontológica del curso de Especialización en Endodoncia de la Facultad São Leopoldo Mandic, Unidad de Belo Horizonte, quejándose de síntomas dolorosos en la región de los dientes 14 y 15. Del antecedente de el diente, se informó por parte de la paciente que se sometió a un retratamiento de endodoncia en la misma institución hace aproximadamente 5 meses debido a un tratamiento anterior que no tuvo éxito. Durante el examen clínico se pudo observar la presencia de una fístula en la región vestibular entre los dientes antes mencionados. Luego de la evaluación clínica y radiográfica, se decidió realizar microcirugía endodóntica, dado el retratamiento realizado previamente. El reporte de caso fue aprobado por el Comité de Ética en Investigación y se encuentra registrado con el número CAAE: 22295119.8.0000.5374 y número de dictamen: 3.651.194.Os fatores etiológicos das alterações pulpares, principalmente os de origem biológica, podem desencadear reações inflamatórias nos tecidos perirradiculares. Apesar dos avanços tecnológicos nos tratamentos endodônticos, aumentando a previsibilidade de sucesso, ainda deparamos com casos de infecção persistente. Quando bem indicada e realizada de forma correta, apresentando taxas satisfatórias de sucesso, a cirurgia parendodôntica surge como uma opção de procedimento complementar à desinfecção endodôntica. O presente estudo, tem por objetivo relatar um caso clínico utilizando-se do conceito de microcirurgia parendodôntica em um caso de periodontite apical persistente. Paciente do gênero feminino, 39 anos de idade, sem quadro de alteração sistêmica, procurou a clínica odontológica do curso de Especialização de Endodontia da Faculdade São Leopoldo Mandic, Unidade Belo Horizonte, queixando-se de sintomatologia dolorosa na região dos elementos dentários 14 e 15. Da história pregressa do dente, foi relatado pela paciente ter realizado o retratamento endodôntico na mesma instituição acerca de 5 meses devido a um tratamento anterior que não foi bem-sucedido. Durante o exame clínico foi possível observar a presença de fístula em região vestibular entre os dentes supracitados. Após avaliação clínica e radiográfica optou-se pela realização da microcirurgia parendodôntica, visto o retratamento realizado anteriormente.
