Life narratives of peolple with chronic kidney insufficiency: self-care and coping mechanism

Resumo: Chronic Kidney Disease is characterized by a progressive and irreversible loss of the kidney's glomerular, tubular and endocrine function. Considered one of the most serious of all chronic diseases due to injuries, physical and psychological damage and consequences for people's daily lives, influencing their self-care.Objectives: To identify in the life narratives of people undergoing conservative treatment of chronic renal failure how the diagnosis was discovered; and how they project their needs for self-care and coping mechanisms. Method: Descriptive, qualitative and Life Narrative, based on the theoretical framework of Nola Pender. Convenience sample including 14 people with chronic kidney disease, at a university institution. Open interview with thematic analysis. Results: Different experiences were evidenced in relation to the discovery of Renal Insufficiency Chronic, with the need for changes in the way of life. The coping mechanisms arose from changes in diet supported by family and religious belief. Final considerations: The discovery of the illness occurred at random, associated with other pathologies or signs and symptoms. The whole process of illness was marked by pain, suffering in different ways, distrust of the diagnosis and denial of the disease. As for projecting their needs for self-care and coping mechanisms, the results showed the urgency of a more human service, given the difficulty of access, waiting time and the search for care. The nurse is essential in creative practices to influence Health Promotion, helping to reframe negative experiences and favoring the reinsertion of these individuals in the society in which they live.

Considerada una de las enfermedades crónicas más graves debido a lesiones, daños físicos y psicológicos y consecuencias para la vida diaria de las personas, que influyen en su autocuidado. Objetivos: Identificar en las narrativas de vida de personas en tratamiento conservador de insuficiencia renal crónica cómo se descubrió el diagnóstico; y cómo proyectan sus necesidades de autocuidado y mecanismos de afrontamiento. Método: Narrativa descriptiva, cualitativa y de vida, basada en el marco teórico de Nola Pender. Muestra de conveniencia que incluye 14 personas con enfermedad renal crónica, en una institución universitaria. Entrevista abierta con análisis temático. Resultados: Se evidenciaron diferentes experiencias en relación al descubrimiento de la IRC, con la necesidad de cambios en la forma de vida. Los mecanismos de afrontamiento surgieron de cambios en la dieta apoyados por creencias familiares y religiosas. Consideraciones finales: El descubrimiento de la enfermedad se produjo al azar, asociado a otras patologías o signos y síntomas. Todo el proceso de la enfermedad estuvo marcado por el dolor, el sufrimiento de diferentes formas, la desconfianza en el diagnóstico y la negación de la enfermedad. En cuanto a proyectar sus necesidades de autocuidado y mecanismos de afrontamiento, los resultados mostraron la urgencia de un servicio más humano, dada la dificultad de acceso, el tiempo de espera y la búsqueda de cuidados. La enfermera es fundamental en las prácticas creativas para incidir en la Promoción de la Salud, ayudando a replantear las experiencias negativas y favoreciendo la reinserción de estos individuos en la sociedad en la que viven.A Doença Renal Crônica é caracterizada por uma perda progressiva, e irreversível, da função glomerular, tubular e endócrina dos rins. Considerada uma das mais graves de todas as doenças crônicas em função dos agravos, danos físicos, psicológicos e consequências para a vida cotidiana das pessoas influenciando no seu autocuidado. Objetivos: Identificar nas narrativas de vida de pessoas em tratamento conservador da insuficiência renal crônica como se deu a descoberta do diagnóstico; e como elas projetam suas necessidades de autocuidado e mecanismos de enfrentamento. Método: Descritivo, qualitativo e Narrativa de Vida, fundamentando-se no referencial teórico de Nola Pender. Amostra de conveniência contemplando 14 pessoascom doença renal crônica, em instituição universitária. Entrevista aberta com realização de análise temática. Resultados: Foram evidenciadas diferentes experiências em relação à descoberta da insuficiência renal crônica, com necessidade de mudanças no seu modo de viver. Os mecanismos de enfrentamentos surgiram das alterações da alimentação apoiadas pela família e crença religiosa.Considerações finais: A descoberta do adoecimento ocorreu ao acaso, associada a outras patologias ou sinais e sintomas. Todo o processo do adoecimentofoi marcado pela dor, sofrimento de diversas formas, desconfiança do diagnóstico e negação da doença. Quanto a projetar suas necessidades de autocuidado e mecanismos de enfrentamento, os resultados evidenciaram a premência de um atendimento mais humano, frente à dificuldade de acesso, ao tempo de espera ea busca de atendimento.
