Approaches to reporting incidents and adverse events in large hospitals: Integrative review

Resumo: It is a case of an integrative review of the approaches used to report incidents and adverse events in hospitalized patients, aiming to: conduct an integrative review of incident and adverse event reporting approaches adopted in large hospitals. Integrative review study with qualitative approach, descriptive in nature. Data were collected in Latin American and Caribbean Literature in Health Sciences, PubMed databases, in the time frame from 2011 to 2021 and submitted to analysis from thematic categories. Results: Three thematic categories emerged: Tools and approaches that permeate the Notification of Incidents and Adverse Events; Contributing Factors for Underreporting of Incidents and Adverse Events in the Hospital Environment; Knowledge of Incidents and Adverse Events. It was identified that the use of a handwritten or online notification form is used on a large scale, to the detriment of the use of medical charts review; the joint work of the Patient Safety Center with the training sector is urgent and necessary; there is a need for decentralisation of reporting of adverse events and events by nursing. The factors contributing to underreporting were based on fear of blaming on the part of institutions. As to the incidence of adverse events, errors in medication administration, were highlighted, followed by pressure injury. Conclusions: It is necessary to have permanent education regarding the notification of adverse events, with implementation of patient safety protocols and processes.

Estudio de revisión integradora con un enfoque cualitativo, de naturaleza descriptiva. Los datos se recopilaron en las bases de datos Literatura Latinoamericana y del Caribe en Ciencias de la Salud, PubMed, en el plazo de 2011 a 2021 y se sometieron a análisis de categorías temáticas. Resultados: Surgieron tres categorías temáticas: Ferrramentos y enfoques que impregnan la notificación de incidentes y eventos adversos; Factores que contribuyen al subregistro de incidentes y eventos adversos en el entorno hospitalario; Conocimiento de incidentes y eventos adversos. Se identificó que el uso de un formulario de notificación escrito a mano o en línea se utiliza a gran escala, en detrimento del uso de la revisión de registros médicos; el trabajo conjunto del Centro de Seguridad del Paciente con el sector de la capacitación es urgente y necesario; existe la necesidad de descentralización de la notificación de incidentes y eventos adversos por parte de la enfermería. Los factores que contribuyen a la subnotificación se basaron en el miedo a culpar por parte de las instituciones. En cuanto a la incidencia de eventos adversos, se destacaron errores en la administración de medicamentos, seguidos de lesiones por presión. Conclusiones: Existe la necesidad de una educación permanente sobre la notificación de eventos adversos, con la implementación de protocolos y procesos de seguridad del paciente.Trata-se de uma revisão integrativa quanto às abordagens utilizadas para notificação de incidentes e eventos adversos em pacientes hospitalizados, tendo como objetivo realizar uma revisão integrativa de abordagens de notificação de incidentes e eventos adversos adotadas em hospitais de grande porte. Estudo de revisão integrativa com abordagem qualitativa, de natureza descritiva. Os dados foram coletados nas bases Literatura Latino-Americana e do Caribe em Ciências da Saúde, PubMed, no recorte temporal de 2011 a 2021 e submetidos à análise a partir de categorias temáticas. Resultados: Emergiram três categorias temáticas: Ferrramentas e abordagens que permeiam a notificação de incidentes e eventos adversos; Fatores contribuintes para a subnotificação de incidentes e eventos adversos no ambiente hospitalar; Conhecimento de incidentes e eventos adversos. Identificou-se a utilização em larga escala de ficha de notificação manunscrita ou on-line em detrimento da revisão de prontuários; o trabalho do Núcleo de Segurança do Paciente junto ao setor de treinamento é urgente e necessário; a necessidade da descentralização da notificação de incidentes e eventos adversos pela enfermagem. Os fatores contribuintes para a subnotificação basearam-se no medo da culpabilização por parte das instituições. Quanto à incidência de eventos adversos, destacaram-se os erros de administração de medicamentos, seguido de lesão por pressão.
