Multivisceral transplantation: a multifactorial approach

Resumo: | Multivisceral transplantation is the only way to restore nutrition through the digestive tract of patients who suffer from intestinal failure or who have inadequate absorption of macro and micronutrients. Most of these transplants occur in the pediatric population, mainly caused by short bowel syndrome, which disables the child's intestines to secrete fluid and absorb nutrition and energy efficiently. The reality of Brazilian patients who have this injury is very harsh, as the Unified Health System (SUS) and the private network do not offer multivisceral transplantation due to the lack of professionals who master the technique of this procedure. The study made use of scientific publications dated between 2015 and 2021 present in the SciElo, PubMed, Up To Date, Medical News Today, Google Scholar, New England Journal of Medicine and ABTO (Brazilian Association of Organ Transplantation) databases in Portuguese. and English addressing the topic “Multivisceral Transplantation”. Research projects, books, case reports and editorials were excluded from this review. From the analysis of the literature found, it was found that although rejection is still quite frequent, several advances have allowed the clinical application of multivisceral transplantation, such as the emergence of new immunosuppressive drugs, inducers, monoclonal and antilymphocytic antibodies. It is therefore concluded that, initially, this type of transplant did not present satisfactory results, but with the improvement of surgical techniques and infection control, the multivisceral transplant appears to be quite promising. |

La mayoría de estos trasplantes ocurren en la población pediátrica, causados principalmente por el síndrome del intestino corto, que inhabilita los intestinos del niño para secretar líquidos y absorber nutrientes y energía de manera eficiente. La realidad de los pacientes brasileños que tienen esta lesión es muy dura, ya que el Sistema Único de Salud (SUS) y la red privada no ofrecen el trasplante multivisceral debido a la falta de profesionales que dominen la técnica de este procedimiento. El estudio hizo uso de publicaciones científicas con fecha entre 2015 y 2021 presentes en las bases de datos SciElo, PubMed, Up To Date, Medical News Today, Google Scholar, New England Journal of Medicine y ABTO (Asociación Brasileña de Trasplante de Órganos) en portugués e inglés. abordando el tema “Trasplante Multivisceral”. Se excluyeron de esta revisión proyectos de investigación, libros, informes de casos y editoriales. Del análisis de la literatura encontrada, se encontró que si bien el rechazo aún es bastante frecuente, varios avances han permitido la aplicación clínica del trasplante multivisceral, como el surgimiento de nuevos fármacos inmunosupresores, inductores, anticuerpos monoclonales y antilinfocíticos. Por tanto, se concluye que, inicialmente, este tipo de trasplante no presentaba resultados satisfactorios, pero con la mejora de las técnicas quirúrgicas y el control de infecciones, el trasplante multivisceral parece ser bastante prometedor.O transplante multivisceral é a única maneira de restabelecer a nutrição pelo trato digestivo de pacientes que sofrem de falência intestinal ou que apresentem absorção inadequada de macro e micronutrientes. A maioria desses transplantes ocorre na população pediátrica, causada principalmente pela síndrome do intestino curto, que incapacita o intestino da criança de secretar fluidos e absorver nutrição e energia eficiente. A realidade dos pacientes brasileiros que apresentam essa injúria é muito dura, pois o Sistema Único de Saúde (SUS) e a rede privada não oferecem o transplante multivisceral pela falta de profissionais que dominem a técnica deste procedimento. O estudo fez o uso de publicações científicas datadas entre 2015 e 2021 presentes nas bases de dados SciElo, PubMed, Up To Date, Medical News Today, Google Scholar, New England Journal of Medicine e ABTO (Associação Brasileira de Transplante de Órgãos) em português e inglês abordando o tema “Transplante Multivisceral”. Foram excluídos desta revisão projetos de pesquisa, livros, relatos de caso e editoriais. A partir da análise das literaturas encontradas, verificou-se que embora a rejeição ainda seja bastante frequente, diversos avanços permitiram a aplicação clínica do transplante multivisceral, como o surgimento de novas drogas imunossupressoras, indutoras, anticorpos antilinfocíticosmono e policlonais.
