Socket Shield Technique associated with immediate implant rehabilitation in an aesthetic zone: A case report

Resumo: An inherent consequence after tooth extraction is the resorption of the alveolar bone, especially that of the buccal bone plate, which leads to a height and width reduction of the alveolar ridge. This aesthetically affects the rehabilitation, and serves as a major challenge in implant dentistry. Several techniques were developed with the goal of containing this resorption, such as immediate implant, bone graft and accelerated tissue regeneration through the use of membranes. However, a more recent alternative called Socket Shield Technique (SST) has demonstrated very promising results, namely the unique capacity of buccal bone plate preservation, which optimizes the aesthetic aspect of the rehabilitation. It consists in retention of a buccal segment of the root in the alveolus, acting as a shield for the implant. The goal of this study is to describe a case report with the use of the SST, associated with the installation of an implant in the area pertaining to unit 21, of a patient with indication for extraction of this unit and implant rehabilitation, due to fracture on the middle third of the tooth. After 7 months, the definitive restoration was made and with a 10 months total proservation, no alteration that indicated functional or aesthetic failure was observed. Results found by this case demonstrates the potential of the SST, however, more studies of higher academic relevance and longer follow-up time must be developed, so that the efficiency of this technique can be truly verified, especially concerning long-term results.

Esto afecta estéticamente a rehabilitación y constituye un gran desafío en implantología. Con el objetivo de contener esta reabsorción se desarrollaron varias técnicas, como implante inmediato, injerto óseo y regeneración tisular acelerada mediante uso de membranas. Pero, una alternativa más reciente llamada Socket Shield Technique (SST) ha demostrado resultados muy prometedores, concretamente la capacidad única de conservación de la placa ósea bucal, que optimiza el aspecto estético de la rehabilitación. Consiste en retención de un segmento bucal de raíz en el alvéolo, actuando como escudo para el implante. El objetivo de este estudio es describir un reporte de caso con uso de la SST, asociada a la instalación de un implante en área perteneciente a la unidad 21, de un paciente con indicación de extracción de esta unidad e rehabilitación con implante, por fractura en el tercio medio del diente. Luego de 7 meses se realizó la restauración definitiva y con una conservación total de 10 meses no se observó ninguna alteración que indicarán falla funcional o estética. Los resultados encontrados en este caso demuestran el potencial de la SST, aunque, se deben desarrollar más estudios de mayor relevancia académica y mayor tiempo de seguimiento, para que realmente se pueda verificar la eficiencia de esta técnica, especialmente en lo que respecta a resultados a largo plazo.Uma consequência pós-extração dentária é a reabsorção do osso alveolar, principalmente da tábua óssea vestibular, o que leva à redução na altura e largura óssea do rebordo alveolar. Isso afeta a reabilitação do ponto de vista estético, o que representa um grande desafio para a implantodontia. Diversas técnicas foram desenvolvidas com o objetivo de conter essa reabsorção, como o implante imediato, enxertos ósseos e o controle do reparo tecidual através de membranas. Mas, uma alternativa mais recente denominada Técnica Socket Shield (TSS) tem demonstrado resultados mais promissores, em especial, a capacidade única de preservar a tábua óssea vestibular, o que otimiza o aspecto estético da reabilitação. Essa técnica consiste em manter um fragmento do terço vestibular da raiz no alvéolo, atuando como um “escudo” para o implante. O objetivo deste trabalho é descrever um relato de caso clínico com o uso da TSS, associada à instalação do implante na área referente à UD 21, de um paciente com indicação de exodontia desta unidade e reabilitação com implante devido à fratura no terço médio do dente. Após 7 meses, a reabilitação definitiva foi realizada e com um total de 10 meses de proservação, nenhuma alteração que indicasse fracasso funcional ou estético foi observada.
