Urinary tract infection: etiology, sensitivity patterns and antimicrobial resistance at a pediatric hospital

Resumo: This study was developed with the objective to identify the bacterial agents of the urinary tract infection, in the pediatric age group, in a Children´s Hospital of Serra Catarinense, distinguish the incidence of urinary tract infection and specify the etiological agent according to age and sex and analyze the profile of sensitivity and resistance. Methodologically, it is characterized by retrospective and cross-sectional study, through consult in the Hospital's database using the procedure code: “treatment of other diseases of the urinary tract” of patients admitted in 2018, selected 52 patients. The following were evaluated: microbial etiology; sensitivity and resistance to antibiotics. As results was twenty-four positive urocultures, twenty female and four male. The most frequent uropathogen was Escherichia coli (70.8%), and its resistance profile was shown to be high for five antimicrobials. It was observed compatibility of research data and literature in relation to sex; and disagreements regarding sex/etiologic agent and age group. There is a higher prevalence of Proteus in females; and greater number of cases in the age group of infants in the study, disagreeing with the literature. Regarding the sensitivity and resistance profile of parenteral antimicrobials, ceftriaxone is in accordance with the literature. The research points to greater resistance regarding the oral use of amoxicillin with clavulanate, SMT-TMP and nitrofurantoin, differing from the literature.

Metodológicamente, se caracteriza por un estudio retrospectivo y transversal a través de la consulta en la base de datos del Hospital utilizando el código de procedimiento: “tratamiento de otras enfermedades del tacto urinario” de pacientes ingresados en 2018, seleccionados 52 pacientes. Se avaluaron: etiología microbiana y resistencia a los antibióticos. Como resultado, tenían veinticuatro urocultivos positivos, veinte de ellos mujeres y cuatro hombres. El uropatógeno más frecuente fue Escherichia coli (70,8%), y se demostró que su patrón de resistencia era alto para cinco antimicrobianos. Se observó la compatibilidad de los datos de investigación y de la literatura en relación con el sexo más afectado y las divergencias con respecto al sexo/agente etiológico y grupo de edad. Hay una mayor prevalencia de Proteus en las mujeres; y mayor número de casos en el grupo de edad de los lactantes en el estudio, en desacuerdo con la literatura. Con respecto al patrón de sensibilidad y resistencia de antimicrobiano parenteral, a ceftriaxona está de acuerdo con la literatura.  La investigación muestra una mayor resistencia con respecto al uso oral de amoxicilina-clavulánico, SMT-TMP y nitrofurantoína, que difiere de la literatura.Este estudo foi desenvolvido com o objetivo de identificar os agentes etiológicos da ITU, na faixa etária pediátrica, em Hospital Infantil da Serra Catarinense, além de distinguir a incidência da ITU entre os sexos, bem como analisar o perfil de sensibilidade e resistência dos antimicrobianos. Metodologicamente caracteriza-se por estudo transversal e retrospectivo, mediante consulta no banco de dados do Hospital utilizando código de procedimento: “tratamento de outras doenças do aparelho urinário” de pacientes internados no ano de 2018. Foram avaliados: etiologia microbiana; sensibilidade e resistência aos antibióticos. Como resultado, foram selecionados 52 pacientes que tiveram vinte e quatro uroculturas positivas, sendo vinte do sexo feminino e quatro do sexo masculino. O uropatógeno mais frequente foi Escherichia coli (70,8%) e seu perfil de resistência demonstrou-se elevado para cinco antimicrobianos. Concluindo, observou-se compatibilidade dos dados da pesquisa e da literatura em relação ao sexo mais acometido, e divergências quanto à agente etiológico e faixa etária. Há maior prevalência de Proteus em sexo feminino; e maior número de casos na faixa etária de lactentes. A pesquisa aponta maior resistência quanto ao uso oral de amoxicilina com clavulanato, sulfametoxazol+trimetropim (SMT-TMP) e nitrofurantoína.
