Surgical approach to compound odontoma in the jaw

Resumo: Odontoma is a benign odontogenic tumor of epithelial and mesenchymal origin, often associated with embedded teeth, dental germs or supernumerary teeth. A classification of odontoma is based on its morphology and can be evaluated as a compound when it has multiple denticles. Treatment consists of surgical excision of the tumor, preserving the related dental units. Therefore, this study aims to report a surgical approach to an odontoma composed by means of a descriptive, qualitative, case report type. Female patient, 13 years old, with tomographic examination showing radiopaque mass structures similar to teeth incorporated in the region of tooth 43. On physical examination, she presented a slight increase in volume in the 43 lingual region, causing dental crowding. The proposed treatment was enucleation and curettage under local anesthesia. The patient returned to the outpatient clinic OMFS after 8 months with panoramic radiography suggesting new bone formation in the region of tooth 43 and without clinical and radiographic signs of lesion recurrence. Thus, odontomas are tumors usually diagnosed in routine radiographic examinations and can be associated with dental retention and crowding. The treatment of choice consists of surgical excision through enucleation associated with curettage, with a low recurrence rate.

La clasificación del odontoma se basa en su morfología y se puede clasificar como compuesto cuando tiene múltiples dentículos. El tratamiento consiste en la escisión quirúrgica del tumor, conservando las unidades dentales relacionadas. Por tanto, este estudio tiene como objetivo reportar el abordaje quirúrgico de un odontoma compuesto por medio de un relato de caso tipo descriptivo, cualitativo. Paciente de sexo femenino, 13 años, con examen tomográfico que muestra masa radiopaca conteniendo estructuras similares a dientes localizadas en la región de lo diente 43. Al examen físico presenta un ligero aumento de volumen en la 43 región lingual, provocando apiñamiento dentario. El tratamiento propuesto fue la enucleación y legrado bajo anestesia local. El paciente regresa a la consulta externa del CTBMF a los 8 meses con radiografía panorámica que sugiere neoformación ósea en la región del diente 43 y sin signos clínicos y radiográficos de recidiva de la lesión. Por lo tanto, los odontomas son tumores que generalmente se diagnostican en exámenes radiográficos de rutina y pueden asociarse con retención y apiñamiento dentario. Diante disso, esse estudo tem o objetivo de relatar a abordagem cirúrgica de um odontoma composto por meio de um estudo descritivo, qualitativo, do tipo relato de caso. Paciente sexo feminino, 13 anos de idade, com exame tomográfico evidenciando massa radiopaca contendo estruturas semelhantes à dentes localizadas na região do dente 43. Ao exame físico apresentava discreto aumento de volume na região lingual do 43 provocando apinhamento dental. O tratamento proposto foi enucleação e curetagem sob anestesia local. Paciente retornou ao ambulatório CTBMF após 8 meses com radiografia panorâmica sugerindo neoformação óssea na região do dente 43 e sem sinais clínicos e radiográficos de recidiva da lesão. Dessa forma, os odontomas são tumores geralmente diagnosticados em exames radiográficos de rotina e podem estar associados a retenções e apinhamentos dentários.
