Association between angiotensin-converting enzyme polymorphisms and the behavior of heart rate variability in healthy individuals: A systematic review

Resumo: The Angiotensin-Converting Enzyme (ACE) generates angiotensin 2 as a final product, which promotes increased sympathetic activity, but the effects of the different ACE polymorphisms on heart rate variability (HRV) is unclear. The purpose of the present review is to synthesize findings in ACE associations and behavior of HRV in healthy subjects. Pubmed, Web of Science, and Scopus databases were searched without language restriction until January 2021. Original studies that analyzed the association between ACE polymorphisms and HRV in healthy individuals were included and review studies, letters to the editor, editorials and points of view were excluded. As the systematic result identified 548 studies as eligible after application of the methodological descriptors. Following the selection process, the studies were evaluated within the inclusion and exclusion criteria and then, six studies were selected for the present review. Paper’s quality was assessed by Q-Genie and all were considered highest quality. Evidence on the impact of ACE polymorphisms on HRV is not consensual, studies do not clearly demonstrate a pattern of HRV behavior due ACE polymorphisms. Based on limited research number and their conflicting results, it is concluded that there is no possibility to speak causally about the associations between ACE polymorphisms and HRV behavior.

El objetivo del presente estudio fue revisar sistemáticamente la evidencia sobre las asociaciones entre los polimorfismos de la ECA y el comportamiento de la VFC en sujetos sanos. Se consultaron las bases de datos Pubmed, Web of Science y Scopus sin restricción idiomática hasta enero de 2021. Se incluyeron estudios originales que analizaban la asociación entre polimorfismos de ECA y VFC en individuos sanos y excluyendo estudios de revisión, cartas al editor, editoriales y puntos de vista. Después de aplicar los descriptores, se consideraron elegibles 548 estudios. En la siguiente etapa del proceso de selección, estos estudios fueron evaluados dentro de los criterios de inclusión y exclusión de la presente revisión y, luego, seis estudios se consideraron capaces de componer la presente revisión. La calidad de los artículos fue evaluada por Q-Genie y todos se consideraron de buena calidad. La evidencia sobre el impacto de los polimorfismos de la ECA en la VFC no es consensuada, Los estúdios no demuestran claramente um patrón de comportamiento de la VFC debido a los polimorfismos de la ECA. Con base en el pequeño número de investigaciones y sus resultados contradictorios, no hay posibilidad de abordar las asociaciones entre los polimorfismos de ECA y el comportamiento de la VFC de manera oportuna.A enzima conversora da angiotensina (ECA) gera como produto final a angiotensina 2 que promove aumento da atividade simpática, porém pouco se sabe sobre os efeitos dos diferentes polimorfismos dessa enzima na variabilidade da frequência cardíaca (VFC). O objetivo do presente estudo foi revisar de forma sistemática as evidências a respeito das associações entre os polimorfismos da ECA e o comportamento da VFC em sujeitos saudáveis. Foram consultadas as bases de dados Pubmed, Web of Science e Scopus sem restrição idiomática até janeiro de 2021. Foram incluídos estudos originais que analisaram a associação entre os polimorfismos da ECA e a VFC em indivíduos saudáveis e excluídos estudos de revisão, cartas ao editor, editoriais e pontos de vista. Após a aplicação dos descritores, 548 estudos foram considerados elegíveis. Na etapa seguinte do processo de seleção esses estudos foram avaliados dentro dos critérios de inclusão e exclusão da presente revisão e, então, seis estudos foram considerados aptos a compor a presente revisão. A qualidade dos artigos foi avaliada pelo Q-Genie e todos foram considerados de boa qualidade. As evidências sobre o impacto dos polimorfismos da ECA na VFC não são consensuais, os estudos não demonstram claramente um padrão de comportamento da VFC em função dos polimorfismos da ECA.
