The use of Karate as a resource in the rehabilitatory process of a patient with sequels of cerebral vascular accident (CVA): an experience report in Occupational Therapy

Resumo: This article aims to report the experience of Occupational Therapy students in the intervention of the case of a male patient, suffering from hemiplegia and hemiparesis resulting from two strokes. The interventions took place during the practice of a curricular component of the 5th semester of the Occupational Therapy course at the Universidade do Estado do Pará (UEPA). The patient was selected from the waiting list of patients with demands for neurological sequelae, trauma-orthopedics and rheumatic diseases, of the Physiotherapy and Occupational Therapy Unit (UEAFTO). The sessions were held twice a week, lasting 50 minutes, from August to November 2021. We chose to intervene with combined models such as the principles of Brain Plasticity, Restriction Therapy and Movement Induction and the technique of mirror therapy. In the interventions, Karate was used as a resource, as well as adaptations for Activities of Daily Living (ADL), and auxiliary adaptations for carrying out activities in the occupational therapy sections. The patient showed enthusiasm and greater engagement with the inclusion of karate in occupational therapy sessions, in addition, it was empirically observed that the patient presented a gain in Range of Motion in the Upper and Lower Limbs (MMSS/LL), ataxia was alleviated presenting greater stability in the MSE and gain of muscular strength. The combination of approaches and techniques added to the rescue of previous occupations contributed to positive results, in addition to improving the students' clinical reasoning and academic experience.

Las intervenciones ocurrieron durante la práctica de un componente curricular del 5º semestre del curso de Terapia Ocupacional en la Universidade do Estado do Pará (UEPA). El paciente fue seleccionado de la lista de espera de pacientes con demanda por secuelas neurológicas, trauma-ortopedia y enfermedades reumáticas, de la Unidad de Fisioterapia y Terapia Ocupacional (UEAFTO). Las sesiones se realizaron dos veces por semana, con una duración de 50 minutos, de agosto a noviembre de 2021. Optamos por intervenir con modelos combinados como los principios de Plasticidad Cerebral, Terapia de Restricción e Inducción del Movimiento y la técnica de la terapia del espejo. En las intervenciones se utilizó como recurso el Karate, además de adaptaciones para Actividades de la Vida Diaria (AVD), y adaptaciones auxiliares para la realización de actividades en las secciones de terapia ocupacional. El paciente mostró entusiasmo y mayor compromiso con la inclusión del karate en las sesiones de terapia ocupacional, además, se observó empíricamente que el paciente presentó una ganancia en el Rango de Movimiento de los Miembros Superiores e Inferiores (MMSS/LL), se alivió la ataxia presentando mayor estabilidad en el MSE y ganancia de fuerza muscular. La combinación de enfoques y técnicas sumadas al rescate de ocupaciones anteriores contribuyó a resultados positivos, además de mejorar el razonamiento clínico y la experiencia académica de los estudiantes.O presente artigo objetiva relatar a experiência de discentes de Terapia Ocupacional na intervenção do caso de um paciente do sexo masculino, acometido de hemiplegia e hemiparesia decorrentes de dois acidentes vascular encenfálico. As intervenções ocorreram durante a prática de um componente curricular do 5º semestre do curso Terapia Ocupacional da Universidade do Estado do Pará (UEPA). O paciente foi selecionado da lista de espera de pacientes com demandas de sequelas neurológicas, traumato-ortopedia e doenças reumáticas, da Unidade de Fisioterapia e Terapia Ocupacional (UEAFTO). As sessões foram realizadas duas vezes por semana, com duração de 50 minutos, nos meses de agosto a novembro de 2021. Optou-se por intervir com modelos combinados como os princípios da Plasticidade Cerebral, Terapia de Restrição e Indução de Movimento e da técnica de terapia espelho. Nas intervenções, utilizou-se como recurso o Karatê, bem como adaptações para Atividades de Vida Diária (AVD), e adaptações auxiliares para execução de atividades nas seções de terapia ocupacional.
