Tratamento não farmacológico da Hipertensão Arterial em pacientes atendidos na Atenção Primária da cidade de Imperatriz-MA

Resumo: Introduction: The treatment of arterial hypertension has two aspects, pharmacological and non-pharmacological. Both are not dissociated, and in certain cases, the non-pharmacological treatment can prevent the progression and establishment of the disease when the individual is classified as prehypertensive. Objective: to evaluate adherence, as well as the effectiveness of non-pharmacological treatment in hypertensive and pre-hypertensive patients, in patients treated in primary care, in the city of Imperatriz-MA. Methodology: This is a before-and-after study, with an intervention proposal. We evaluated 679 medical records of patients at the Beira Rio Basic Health Unit in Imperatriz, MA. Of these, 145 were visited and 18 accepted the invitation to participate in the research. The inclusion criteria were: being hypertensive or pre-hypertensive, being between 20 and 70 years old, willing to submit to the questionnaire, collection of anthropometric measurements, vital signs and the attempt to adapt to the intervention measures proposed by the team. A self-designed questionnaire was applied and blood pressure monitoring cards were provided. At the time of the visit for collection, an approach was made to what arterial hypertension was and ways to obtain better control based on changes in lifestyle habits. Results: Adherence to non-pharmacological treatment was extremely low, even in those patients with difficult control of blood pressure values ​​with use of more than one medication and associated comorbidities. Conclusion: Changes in life habits face socioeconomic and cultural factors as a barrier, which undermines one of the pillars of the treatment of Systemic Arterial Hypertension.

Ambos no están disociados y, en determinados casos, el tratamiento no farmacológico puede evitar la progresión y establecimiento de la enfermedad cuando el individuo se clasifica como prehipertenso. Objetivo: evaluar la adherencia, así como la efectividad del tratamiento no farmacológico en pacientes hipertensos y prehipertensos, en pacientes atendidos en atención primaria, en la ciudad de Imperatriz-MA. Metodología: Se trata de un estudio de antes y después, con una propuesta de intervención. Se evaluaron 679 historias clínicas de pacientes de la Unidad Básica de Salud Beira Rio de Imperatriz, MA. De estos, 145 fueron visitados y 18 aceptaron la invitación a participar de la investigación. Los criterios de inclusión fueron: ser hipertenso o prehipertenso, tener entre 20 y 70 años, disposición a someterse al cuestionario, toma de medidas antropométricas, signos vitales e intento de adaptación a las medidas de intervención propuestas por el equipo. Se aplicó un cuestionario de diseño propio y se entregaron tarjetas de control de la presión arterial. En el momento de la visita de recolección se realizó un acercamiento a lo que era la hipertensión arterial y las formas de obtener un mejor control en base a cambios en los hábitos de vida. Resultados: La adherencia al tratamiento no farmacológico fue extremadamente baja, incluso en aquellos pacientes con difícil control de los valores de presión arterial con uso de más de un medicamento y comorbilidades asociadas. Conclusión: Los cambios en los hábitos de vida enfrentan como barrera los factores socioeconómicos y culturales, lo que socava uno de los pilares del tratamiento de la Hipertensión Arterial Sistémica.Introdução: O tratamento da hipertensão arterial tem duas vertentes, o farmacológico e o não farmacológico. Ambos não se dissociam, e em determinados casos o não farmacológico pode prevenir a progressão e o estabelecimento da doença quando o indivíduo é classificado em pré-hipertenso. Objetivo: avaliar a adesão, bem como a eficácia do tratamento não farmacológico em hipertensos e pré-hipertensos, em pacientes atendidos na atenção primária, da cidade de Imperatriz-MA. Metodologia: Trata-se de um estudo do tipo antes e depois, com proposta de intervenção. Foram avaliados 679 prontuários dos pacientes da Unidade Básica de Saúde Beira Rio em Imperatriz, MA. Destes, 145 foram visitados e 18 aceitaram o convite para participar da pesquisa. Os critérios de inclusão foram: serem hipertensos ou pré-hipertensos, ter entre 20 e 70 anos de idade, dispostos a se submeterem ao questionário, coleta de medidas antropométricas, sinais vitais e a tentativa de se adequarem as medidas de intervenção propostas pela equipe. Foi aplicado um questionário auto elaborado e fornecido os cartões de acompanhamento da pressão arterial. No momento da visita para coleta foi feita uma abordagem sobre o que era a hipertensão arterial e as formas de se obter um melhor controle a partir das modificações de hábitos de vida. Resultados: A adesão ao tratamento não farmacológico foi extremamente baixa, mesmo naqueles pacientes com um controle difícil dos valores da pressão arterial com uso de mais de um medicamento e comorbidades associadas.
