The use of biomodels in orthognathic surgery: A clinical case report

Resumo: The use of a biomodel allows for simulation and aids in team communication, can be used as prostheses, reduces surgical time, and is a reliable copy of bone tissue. Thus, this work aims to report a clinical case of using the biomodel in orthognathic surgery, in order to demonstrate the benefits obtained with the use of biomodels in surgical planning, such as reducing time, increasing predictability, pre-modeling of titanium plates used in bone fixations. Melanoderma patient, male, sought dental care, with dissatisfaction of facial aesthetics and difficulty in phonation, chewing and breathing. After a detailed physical examination and evaluation through imaging tests, Angle skeletal classification III was identified, with indication for surgical treatment. The trans-operative was simplified and faster due to the previous model of the plates and precision of adaptation of them in the bone surface. The use of biomodels and the previous modeling of fixation plates favored greater surgical precision, through the reduction of trans-operative morbidity and reduction of surgical time. Thus, it emphasizes that this type of approach can be an important ally in the treatment of correction of dento-facial deformities. Furthermore, the knowledge of these new tools enabled a better diagnosis and surgical precision.

Así, este trabajo tiene como objetivo reportar un caso clínico de uso del biomodelo en cirugía ortognática, con el fin de demostrar los beneficios que se obtienen con el uso de biomodelos en la planificación quirúrgica, tales como reducción de tiempo, aumento de la predictibilidad, premodelado de placas de titanio utilizadas en fijaciones óseas. Paciente de sexo masculino con melanoderma que acude al dentista, con insatisfacción de la estética facial y dificultad en la fonación, masticación y respiración. Luego de un examen físico detallado y evaluación mediante pruebas de imagen, se identificó el ángulo esquelético de clasificación III, con indicación de tratamiento quirúrgico. El transoperatorio se simplificó y agilizó debido al modelo anterior de las placas y la precisión de adaptación de las mismas en la superficie ósea. El uso de biomodelos y el modelado previo de placas de fijación favorecieron una mayor precisión quirúrgica, mediante la reducción de la morbilidad transoperatoria y la reducción del tiempo quirúrgico. Así, destaca que este tipo de abordaje puede ser un aliado importante en el tratamiento de la corrección de las deformidades dento-faciales, además, el conocimiento de estas nuevas herramientas permitió un mejor diagnóstico y precisión quirúrgica.O uso de biomodelo permite a simulação e auxilia na comunicação da equipe, pode ser utilizado como próteses, reduz o tempo cirúrgico, sendo a cópia fidedigna do tecido ósseo. Desta forma, este trabalho tem como objetivo relatar um caso clínico de uso do biomodelo em cirurgia ortognática, a fim de demonstrar os benefícios obtidos com o uso dos biomodelos no planejamento cirúrgico, como a redução do tempo, aumento da previsibilidade, pré-modelagem das placas de titânio utilizadas nas fixações ósseas. Paciente melanoderma, sexo masculino, procurou atendimento odontológico, com insatisfação da estética facial e dificuldade na fonação, mastigação e respiração. Após um detalhado exame físico e avaliação através de exames de imagem, identificou-se a classificação III esquelética de Angle, com indicação para tratamento cirúrgico. O trans-operatório foi simplificado e mais rápido por conta da prévia modelam das placas e precisão de adaptação das mesmas na superfície óssea. O uso dos biomodelos e a prévia modelagem das placas de fixação favoreceu a uma maior precisão cirúrgica, através da diminuição da morbidade trans-operatória e redução do tempo cirúrgico.
