Treatment of Covid-19 in a pre-diabetic patient in the intensive care unit: a case report

Resumo: | Coronavirus disease (COVID-19) has been widely and globally studied and researched, as it is a new disease with a varied clinical spectrum and its pathophysiology is not completely known. Moreover, the aggravations are associated with the lifestyle and previous diseases of the patients, and the evolution of each patient. Among these problems are obesity and diabetes, diseases that lead to metabolic disorders in the body. The aim of this report is to report the case of a pre-diabetic patient, obese grade II, with infection caused by the Sars-CoV-2 virus and evolution to diabetes during infection, besides presenting secondary physiological repercussions to the infection. The patient studied was diagnosed with Type 2 Diabetes during hospitalization and presented multiple ground-glass opacities with bilateral multifocal distribution on chest computed tomography (CT), so he was referred to the Intensive Care Unit (ICU). In the ICU, he was positive for SARS-Cov-2 by RT-PCR, Orotracheal Intubation was performed, pharmacotherapy was started, and laboratory tests confirmed that the patient is diabetic. In this case, it is notable that because the patient had comorbidities for COVID-19 - pre-diabetes and grade 2 obesity - the disease may have been aggravated by the factors. Thus, he needed pulmonary rehabilitation and intensive care, such as use of medications used in more advanced cases of the disease and ICU monitoring. Therefore, we conclude that it is essential for health professionals to know the clinical picture and pre-existing pathologies of the affected patients, because this data will offer the professional the choice of individualized therapies. |

Además, los agravamientos están asociados a los hábitos de vida y a las enfermedades previas de los pacientes y a la evolución de cada uno de ellos. Entre estos agravios, se encuentran la obesidad y la diabetes, enfermedades que provocan alteraciones metabólicas en el organismo. El objetivo de este informe es reportar el caso de un paciente prediabético, obeso grado II con infección causada por el virus Sars-CoV-2 y evolución a diabetes durante la infección, además de presentar repercusiones fisiológicas secundarias a la infección. El paciente estudiado fue diagnosticado de diabetes tipo 2 durante su hospitalización y presentó múltiples opacidades pulmonares en vidrio deslustrado con distribución multifocal bilateral en la tomografía computarizada (TC) de tórax, por lo que fue remitido a la Unidad de Cuidados Intensivos (UCI). En la UCI, dio positivo para el SARS-Cov-2 mediante RT-PCR, se realizó una intubación orotraqueal, se inició la farmacoterapia y las pruebas de laboratorio confirmaron que el paciente es diabético. En este caso, cabe destacar que, dado que el paciente presentaba comorbilidades para la COVID-19 -prediabetes y obesidad de grado 2-, la enfermedad puede haberse visto agravada por estos factores. Por lo tanto, necesitaba rehabilitación pulmonar y cuidados intensivos, como el uso de medicamentos utilizados en casos más avanzados de la enfermedad y la monitorización en la UCI. Por lo tanto, concluimos que es fundamental que los profesionales sanitarios conozcan el cuadro clínico y las patologías preexistentes de los pacientes afectados, ya que estos datos ofrecerán al profesional la elección de terapias individualizadas.A doença do coronavírus (COVID-19) tem sido ampla e globalmente estudada e pesquisada, por ser uma doença nova e de variado espectro clínico além de sua fisiopatologia não ser completamente conhecida. Ademais, os agravos estão associados aos hábitos de vida, doenças prévias e à evolução de cada paciente. Dentre esses agravos, estão a obesidade e a diabetes, enfermidades que levam a distúrbios metabólicos do organismo. O objetivo desse relato é reportar o caso de um paciente pré-diabético, obeso grau II com infecção causada pelo vírus Sars-CoV-2 e evolução para diabetes durante a infecção, além de apresentar repercussões fisiológicas secundárias à infecção. O paciente estudado recebeu o diagnóstico de Diabetes tipo 2 durante a internação e apresentou múltiplas opacidades pulmonares em vidro fosco com distribuição multifocal bilateral na Tomografia Computadorizada de Tórax (TC de tórax), assim foi encaminhado para Unidade de Terapia Intensiva (UTI). Na UTI, positivou para SARS-Cov-2 através do RT-PCR, foi realizada Intubação Orotraqueal, iniciou-se a farmacoterapia e os exames laboratoriais confirmaram que o paciente é diabético. Neste caso, é notável que, devido ao paciente apresentar comorbidades para a COVID-19 – pré-diabetes e obesidade grau 2 – a doença pode ter sido agravada por esses fatores. Assim, ele precisou de reabilitação pulmonar e cuidados intensivos, como uso de medicações utilizadas em casos mais avançados da doença e monitoramento da UTI.
