Children on outpatient antineoplastic therapy: stressors, parental reaction and interaction in the face of home care

Resumo: Objective: to understand the stressors, reaction and parental interaction of children undergoing outpatient chemotherapy treatment in relation to home care. Method: a comprehensive study, with a qualitative approach, carried out with fourteen fathers and mothers of children undergoing outpatient chemotherapy treatment at a specialized hospital. Data collection took place through a focus group from January to February 2018. Data were submitted to content analysis proposed by Bardin and discussed according to Betty Neuman's Systems Model. Results: three thematic categories were created: 1. Stressors: taking care of the child at home after outpatient chemotherapy; 2. Interaction with the environment: living with cancer and outpatient chemotherapy treatment; and 3. Parents' reaction: overcoming having a child with cancer and undergoing outpatient chemotherapy treatment. Conclusion: for parents, living with cancer, chemotherapy treatment and providing care to the child at home involves suffering and uncertainties. This experience is permeated by stressors in terms of changes in family dynamics and structure, lack of autonomy to play their parental role and insecurity regarding home care. The interaction with the oncopediatric universe allows parents to experience social stigmas of the disease and suffer intense psychological shock and emotional imbalance. However, this experience is softened when parents are faced with the humanization of the health team and with the solidarity of other parents who experience a similar situation, at this moment, parents find strength to face and overcome difficult times.

Método: estudio integral, con abordaje cualitativo, realizado con catorce padres y madres de niños en tratamiento ambulatorio de quimioterapia en un hospital especializado. La recolección de datos ocurrió a través de un grupo focal de enero a febrero de 2018. Los datos fueron sometidos al análisis de contenido propuesto por Bardin y discutidos según el Modelo de Sistemas de Betty Neuman. Resultados: fueron creadas tres categorías temáticas: 1. Estresores: cuidar al niño en casa después de la quimioterapia ambulatoria; 2. Interacción con el entorno: convivencia con el cáncer y tratamiento quimioterápico ambulatorio; y 3. Reacción de los padres: superar tener un hijo con cáncer y someterse a un tratamiento de quimioterapia ambulatorio. Conclusión: para los padres, vivir con cáncer, el tratamiento con quimioterapia y brindar cuidados al niño en el hogar implica sufrimiento e incertidumbres. Esta experiencia está permeada por estresores en cuanto a cambios en la dinámica y estructura familiar, falta de autonomía para desempeñar su rol parental e inseguridad en cuanto al cuidado domiciliario. La interacción con el universo oncopediátrico permite que los padres experimenten estigmas sociales de la enfermedad y sufran un intenso shock psicológico y desequilibrio emocional. Sin embargo, esta experiencia se suaviza cuando los padres se enfrentan con la humanización del equipo de salud y con la solidaridad de otros padres que viven una situación similar, en este momento los padres encuentran fortaleza para enfrentar y superar momentos difíciles.Objetivo: compreender os fatores estressores, reação e interação parental de crianças em tratamento quimioterápico ambulatorial frente ao cuidado domiciliar. Método: estudo compreensivo, de abordagem qualitativa, realizado com quatorze pais e mães de crianças em tratamento quimioterápico ambulatorial de um hospital especializado. A coleta de dados ocorreu por meio de grupo focal no período de janeiro a fevereiro de 2018. Os dados foram submetidos à análise de conteúdo proposta por Bardin e discutidos de acordo com o Modelo de Sistemas de Betty Neuman. Resultados: tem-se a criação de três categorias temáticas: 1. Fatores Estressores: cuidando da criança no domicílio após quimioterapia ambulatorial; 2. Interação com o ambiente: convivendo com o câncer e com o tratamento quimioterápico ambulatorial; e 3. Reação dos pais: superando ter um filho com câncer e em tratamento quimioterápico ambulatorial.  Conclusão: para os pais, conviver com o câncer, tratamento quimioterápico e prestar cuidados à criança no âmbito domiciliar envolve sofrimento e incertezas. Essa experiência é permeada por agentes estressores no que se refere as modificações na dinâmica e estrutura familiar, falta de autonomia para desempenhar seu papel parental e insegurança quanto aos cuidados domiciliares. A interação com o universo oncopediátrico permite que os pais experienciem estigmas sociais da doença e sofram intenso abalo psicológico e desequilíbrio emocional.
