Resumo: | Introduction: Treatment-Resistant Depression (TRD) does not have an absolute consensus on its definition, but can be understood as a severe depressive condition with an unsatisfactory response to two antidepressants from two different classes. Ketamine is a dissociative anesthetic that has an antagonistic action on glutamate/aspartate receptors, therefore, it acts in the pathophysiological condition of depression. Objective: The present study seeks to examine the particularities of the use of ketamines in people who do not improve with conventional treatments for depression. Materials and Methods: This is an integrative literature review about the role of using ketamines in patients refractory to treatment. The PICO strategy was used to prepare the guiding question. Furthermore, the descriptors “Refractory depression” were crossed; “Ketamines”; “Management”, in the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (SCIELO), Ebscohost, Google Scholar and Virtual Health Library (VHL) databases. Results and Discussion: Refractory depression is serious without a clear definition. Both ketamine and electroconvulsive therapy (ECT) are considered satisfactory treatments and do not demonstrate differences in reducing suicidal ideation in these patients. Conclusion: This review highlights that for better management of ESRD it is necessary to formalize the therapeutic approach protocol for this disorder. Furthermore, it reveals that ketamines present superior results to other pharmacological measures and are very similar to ECT, but require follow-up due to the high rate of recurrence. |

La ketamina es un anestésico disociativo que tiene acción antagonista sobre los receptores de glutamato/aspartato, por lo que actúa en el cuadro fisiopatológico de la depresión. Objetivo: El presente estudio busca examinar las particularidades del uso de ketaminas en personas que no mejoran con tratamientos convencionales para la depresión. Materiales y Métodos: Se trata de una revisión integradora de la literatura sobre el papel del uso de ketaminas en pacientes refractarios al tratamiento. Se utilizó la estrategia PICO para preparar la pregunta orientadora. Además, se cruzaron los descriptores “Depresión refractaria”; “Ketaminas”; “Gestión”, en las bases de datos de la Biblioteca Nacional de Medicina (PubMed MEDLINE), Scientific Electronic Library Online (SCIELO), Ebscohost, Google Scholar y Virtual Health Library (BVS). Resultados y Discusión: La depresión refractaria es grave sin una definición clara. Tanto la ketamina como la terapia electroconvulsiva (TEC) se consideran tratamientos satisfactorios y no demuestran diferencias en la reducción de la ideación suicida en estos pacientes. Conclusión: Esta revisión destaca que para un mejor manejo de la ERT es necesario formalizar el protocolo de abordaje terapéutico para este trastorno. Además, revela que las ketaminas presentan resultados superiores a otras medidas farmacológicas y son muy similares a la TEC, pero requieren seguimiento debido a la alta tasa de recurrencia.Introdução: A Depressão Resistente ao Tratamento (DRT) não apresenta um consenso absoluto sobre sua definição, mas pode ser entendida como o quadro depressivo grave com resposta insatisfatória a dois antidepressivos de duas classes diferentes. A cetamina é um dissociativo anestésico que possui ação antagonista dos receptores de glutamato/aspartato, logo, age no quadro fisiopatológico da depressão. Objetivo: O presente estudo busca examinar as particularidades do uso de cetaminas em pessoas que não apresentam melhora com os tratamentos convencionais para a depressão. Materiais e Métodos: Trata-se de uma revisão integrativa de literatura acerca do papel da utilização de cetaminas em pacientes refratários ao tratamento. Utilizou-se a estratégia PICO para a elaboração da pergunta norteadora. Ademais, realizou-se o cruzamento dos descritores “Depressão refratária”; “Cetaminas”; “Manejo”, nas bases de dados National Library of Medicine (PubMed MEDLINE), Scientific Eletronic Library Online (SCIELO), Ebscohost, Google Scholar e Biblioteca Virtual de Saúde (BVS). Resultados e Discussão: A depressão refratária é grave sem definição clara. Tanto a cetamina como a eletroconvulsoterapia (ECT) são consideradas tratamentos satisfatórios e não demonstram diferenças na redução da ideação suicida nesses pacientes. Conclusão: Essa revisão destaca que para o melhor manejo da DRT é preciso formalizar o protocolo de abordagem terapêutico deste transtorno.
