Canine visceral leishmaniasis in the northern region of the state of Rio Grande do Sul

Resumo: Leishmaniasis is a zoonotic disease caused by the protozoan species Leishmania spp., diagnosed infrequently in dogs from the northern region of Rio Grande do Sul (RS). Because of this, allochthonous cases become important for investigation. The objective of this work is to report a case of visceral leishmaniasis in a canine and to emphasize its clinical, diagnostic and therapeutic aspects. A 3-year-old Pug dog from Araranguá, in the state of Santa Catarina (SC), was treated with a main complaint of hyporexia, prostration, onychogryphosis and pain in the pelvic limbs. The patient had been using immunosuppressants for two months due to the presence of polyarthritis. Clinical examination revealed a skin nodule and an increase in popliteal lymph nodes. Fine-needle cytology was performed at both sites, where amastigote forms of Leishmania spp. The case was notified to Organs competent bodies and serology was performed using the Elisa method to confirm the diagnosis. Treatment with miltefosine, domperidone, allopurinol and Leish Tec® vaccine was instituted. The importance of a thorough anamnesis associated with laboratory tests to screen patients with the disease is highlighted, focusing on the possibility of allochthonous cases being diagnosed in non-endemic regions of Brazil.

Debido a esto, los casos alóctonos se vuelven importantes para la investigación. El objetivo de este trabajo es reportar un caso de leishmaniasis visceral en un canino y enfatizar sus aspectos clínicos, diagnósticos y terapéuticos. Un perro Pug de 3 años de Araranguá, en el estado de Santa Catarina (SC), fue tratado con un síntoma principal de hiporexia, postración, onicogrifosis y dolor en los miembros pélvicos. El paciente tomaba inmunosupresores desde hacía dos meses por la presencia de poliartritis. El examen clínico reveló un nódulo en la piel y un aumento en los ganglios linfáticos poplíteos. Se realizó citología con aguja fina en ambos sitios, donde se observaron formas amastigotas de Leishmania spp. El caso fue notificado a los Órganos competentes y se realizó serología por el método Elisa para confirmar el diagnóstico. Se inició tratamiento con miltefosina, domperidona, alopurinol y vacuna Leish Tec®. Se destaca la importancia de una anamnesis completa asociada a los exámenes de laboratorio para el tamizaje de los pacientes portadores de la enfermedad, con foco en la posibilidad de diagnóstico de casos alóctonos en regiones no endémicas de Brasil.A leishmaniose é uma doença zoonótica causada pelo protozoário da espécie Leishmania spp., diagnosticada com pouca frequência em cães da região norte do Rio Grande do Sul (RS). Devido a isso, casos alóctones se tornam importantes para investigação. O objetivo deste trabalho é relatar um caso de leishmaniose visceral em um canino e enfatizar seus aspectos clínicos, diagnósticos e terapêuticos. Foi atendido um cão da raça Pug com 3 anos, proveniente de Araranguá, no estado de Santa Catarina (SC), com queixa principal de hiporexia, prostração, onicogrifose e algia em membros pélvicos. O paciente fazia uso de imunossupressores há dois meses devido a presença de poliartrite. Ao exame clínico, observou-se nódulo cutâneo e aumento de linfonodos poplíteos. Realizada citologia por agulha fina de ambos locais, onde foram observadas formas amastigotas de Leishmania spp. O caso foi notificado aos órgãos competentes e realizado sorologia pelo método de Elisa para confirmação do diagnóstico. Instituiu-se tratamento com miltefosina, domperidona, alopurinol e vacina Leish Tec®.
