Caring or forwarding? Challenges in the diagnosis and management of patients with dementia in Primary Health Care

Resumo: Introduction: With population aging, we have an increase in neurodegenerative conditions, including dementia syndromes. It is essential that health services are able to receive this demand, especially in Primary Health Care, for better monitoring of the elderly. Objective: to know the difficulties faced by doctors of the Family Health Strategy in the city of São Caetano do Sul in the care of patients with dementia. Method: this is a cross-sectional, quantitative, descriptive, exploratory approach, with a convenience sample, not probabilistic. The Brazilian version, validated for physicians, of the instrument “Atenció Sanitària de Les Demències: la visió de L'Atenció Primarià” was used. Results: 100% of the doctors of the Family Health Strategy in the municipality of São Caetano do Sul were evaluated. Regarding the care provided in Primary Health Care, 82.6% of the physicians evaluated reported that more than 25% of the patients seen were elderly. Most participants (64.3%) indicated that the greatest difficulty in identifying a case of dementia was the differentiation of signs and symptoms of dementia from geriatric depression. Another challenge observed by 46.4% of physicians was the differentiation of signs and symptoms of dementia from the physiological process of aging, senescence. Among the main signs and symptoms that lead to suspect the diagnosis of dementia, 46.4% of physicians pointed to cognitive impairment with memory alterations, 28.6% reported that the impairment of the ability to perform activities of daily living would be the first signs and symptoms and 21.4% reported that the presence of psychological and behavioral symptoms is what would most lead to suspicion of the diagnosis. Final Considerations: It was observed that physicians had difficulty in evaluating patients with cognitive complaints in relation to screening for reversible dementias and investigating differential diagnoses with common aging and mood disorders, for example. It is essential to train the studied group to improve the quality of care for the elderly.

Es fundamental que los servicios de salud sean capaces de recibir esta demanda, especialmente en la Atención Primaria de Salud, para un mejor seguimiento de las personas mayores. Objetivo: conocer las dificultades que enfrentan los médicos de la Estrategia de Salud de la Familia en la ciudad de São Caetano do Sul en la atención de pacientes con demencia. Método: se trata de un abordaje transversal, cuantitativo, descriptivo, exploratorio, con muestra por conveniencia, no probabilística. Se utilizó la versión brasileña, validada para médicos, del instrumento “Atenció Sanitària de Les Demències: la visió de L'Atenció Primarià”. Resultados: Fueron evaluados el 100% de los médicos de la Estrategia de Salud de la Familia del municipio de São Caetano do Sul. En cuanto a la atención prestada en la Atención Primaria de Salud, el 82,6% de los médicos evaluados refirieron que más del 25% de los pacientes atendidos eran ancianos. La mayoría de los participantes (64,3%) indicó que la mayor dificultad para identificar un caso de demencia fue la diferenciación de los signos y síntomas de demencia de la depresión geriátrica. Otro desafío observado por el 46,4% de los médicos fue la diferenciación de los signos y síntomas de la demencia del proceso fisiológico del envejecimiento, la senescencia. Entre los principales signos y síntomas que hacen sospechar un diagnóstico de demencia, el 46,4% de los médicos señalaron deterioro cognitivo con alteraciones de la memoria, el 28,6% refirió que el deterioro de la capacidad para realizar las actividades de la vida diaria serían los primeros signos y síntomas y el 21,4% refirió que la presencia de síntomas psicológicos y conductuales es lo que más los llevaría a sospechar el diagnóstico. Consideraciones finales: se observó que los médicos tenían dificultades para evaluar a los pacientes con quejas cognitivas en relación con la detección de demencias reversibles y la investigación de diagnósticos diferenciales con el envejecimiento común y los trastornos del estado de ánimo, por ejemplo. Es fundamental capacitar al grupo estudiado para mejorar la calidad de la atención a las personas mayores.Introdução: Com o envelhecimento populacional, temos o aumento de quadros neurodegenerativos, dentre eles as síndromes demenciais. É fundamental que os serviços de saúde estejam habilitados para receber essa demanda, em especial na Atenção Primária à Saúde, para melhor acompanhamento dos idosos. Objetivo: conhecer as dificuldades enfrentadas pelos médicos da Estratégia da Saúde da Família do município de São Caetano do Sul no atendimento aos pacientes com demência. Método: trata-se de abordagem transversal, quantitativa, descritiva, exploratória, com amostra por conveniência, não probabilística. Foi utilizada a versão brasileira, validada para médicos, do instrumento “Atenció Sanitària de Les Demències: la visió de L' Atenció Primarià. Resultados: Foram avaliados 100% dos médicos da Estratégia da Saúde da Família do município de São Caetano do Sul. Em relação ao atendimento prestado na Atenção Primária à Saúde, 82,6% dos médicos avaliados referiram que mais de 25% dos pacientes atendidos são idosos. A maioria dos participantes (64,3%) apontou como maior dificuldade para identificação de um caso de demência, a diferenciação dos sinais e sintomas da demência da depressão geriátrica. Outro desafio observado por 46,4% dos médicos, foi a diferenciação dos sinais e sintomas de demência do processo fisiológico do envelhecimento, a senescência. Dentre os principais sinais e sintomas que levam a suspeitar do diagnóstico de demência, 46,4% dos médicos apontaram o comprometimento cognitivo com alterações de memória, 28,6% relataram que o comprometimento da capacidade de executar atividades da vida diária seriam os primeiros sinais e sintomas e 21,4% referiram que a presença de sintomas psicológicos e comportamentais é o que mais levaria a suspeitar do diagnóstico. Considerações Finais: Observou-se dificuldade dos médicos para avaliação dos pacientes com queixas cognitivas em relação ao rastreio de quadros de demências reversíveis e investigação de diagnósticos diferenciais com o envelhecimento comum e transtornos de humor, por exemplo.
