Treatment of systemic arterial hypertension: an integrative literature review about Hydrochlorothiazide vs Chlortalidone

Resumo: Objective: To carry out an integrative literature review seeking to analyze the main differences in the use of Hydrochlorothiazide vs Chlorthalidone, in order to understand whether any drug overlaps the other. Methodology: This integrative literature review was carried out, analyzing articles from January 2010 to June 2022, in Portuguese, English and Spanish in the following databases: MEDLINE via PubMed, Europe PMC, Lilacs and SciELO. The descriptors used were “Chlorthalidone”, “Hydrochlorothiazide” and “Hypertension”, according to the DeCS, covering 31 articles at the end. Results: Several studies point to a more intense effect of chlorthalidone on hydrochlorothiazide, but emphasize that the former has greater adverse effects, such as hypokalemia and hyponatremia. Some studies show that the use of chlorthalidone over hydrochlorothiazide brought about a reduction in cardiovascular risk of approximately 18%, a result that has been much debated in the literature as it is the result of an indirect meta-analysis. Conclusion: The literature still differs regarding the use of chlorthalidone over hydrochlorothiazide, requiring further conclusive studies on the direct comparative effect between the two drugs. Literature data converge that chlorthalidone has a more intense effect than hydrochlorothiazide, bringing, however, more adverse effects to the patient, such as hyperkalemia, for example.

Metodología: Se realizó una revisión integrativa de la literatura, analizando artículos desde enero de 2010 hasta junio de 2022, en portugués, inglés y español en las siguientes bases de datos: MEDLINE vía PubMed, Europe PMC, Lilacs y SciELO. Los descriptores utilizados fueron “Clortalidona”, “Hidroclorotiazida” e “Hipertensión”, según el DeCS, cubriendo al final 31 artículos. Resultados: Varios estudios apuntan a un efecto más intenso de la clortalidona sobre la hidroclorotiazida, pero destacan que la primera tiene mayores efectos adversos, como hipopotasemia e hiponatremia. Algunos estudios muestran que el uso de clortalidona sobre hidroclorotiazida produjo una reducción del riesgo cardiovascular de aproximadamente un 18%, resultado que ha sido muy debatido en la literatura al ser el resultado de un metaanálisis indirecto. Conclusión: La literatura aún difiere en cuanto al uso de clortalidona sobre hidroclorotiazida, lo que requiere más estudios concluyentes sobre el efecto comparativo directo entre los dos fármacos. Los datos de la literatura convergen en que la clortalidona tiene un efecto más intenso que la hidroclorotiazida, trayendo, sin embargo, más efectos adversos para el paciente, como la hiperpotasemia, por ejemplo.Objetivo: Realizar uma revisão de literatura integrativa buscando analisar as principais diferenças no uso da Hidroclorotiazida vs Clortalidona, afim de entender se algum medicamento se sobrepõe sobre o outro. Metodologia: Foi realizada uma revisão de literatura integrativa, analisando-se artigos desde janeiro de 2010 até junho de 2022, nos idiomas português, inglês e espanhol nas seguintes bases de dados: MEDLINE via PubMed, Europe PMC, Lilacs e SciELO. Os descritores utilizados foram “Clortalidona”, “Hidroclorotiazida” e “Hipertensão”, de acordo com o DeCS, abrangendo ao final 31 artigos. Resultados: Diversos estudos apontam para o efeito mais intenso da clortalidona sobre a hidroclorotiazida, mas ressaltam que o primeiro possui maiores efeitos adversos, como hipocalemia e hiponatremia. Algumas pesquisas trazem que o uso da clortalidona sobre a hidroclorotiazida trouxe uma redução de risco cardiovascular de aproximadamente 18%, sendo um resultado bastante debatido na literatura por ser fruto de meta-análise indireta.  Conclusão: A literatura ainda diverge quanto ao uso da clortalidona sobre a hidroclorotiazida, necessitando de mais estudos conclusivos sobre o efeito direto comparativo entre as duas drogas.
