Direct laryngoscopy as a diagnostic method of laryngeal paralysis in canine: case report

Resumo: | The larynx is the organ that allows the passage of air to the caudal respiratory tract by controlling the air flow during the breathing process. Laryngeal paralysis consists of the unilateral or bilateral inability of the arytenoid cartilages to abduct during inspiration. It is a disorder that mainly affects elderly dogs of large to giant breeds and with congenital or acquired etiology, especially affecting breeds such as Bouvier des Flandres, Siberian Huskie, Bull Terrier, German Shepherd, Labrador, Golden Retriever, among others. Clinical signs include obstruction of the anterior airways during inspiration, exercise intolerance, inspiratory stridor and changes in vocalization, generating a weak and hoarse bark, in addition to choking and sporadic coughing. The diagnosis begins with clinical signs, physical examination, and is definitive by inspection of the laryngeal function by direct laryngoscopy or with the aid of an endoscope. Supportive treatment consists of stabilizing the patient with oxygen therapy and administering anti-inflammatory drugs. In animals with respiratory distress, emergency therapy should be instituted. Surgical treatment is the choice and consists of stabilizing and repositioning the arytenoid cartilages. This work aims to report a clinical case of a canine affected with laryngeal paralysis and describe the diagnostic investigation through direct laryngoscopy. |

La parálisis laríngea consiste en la incapacidad unilateral o bilateral de los cartílagos aritenoides para abducir durante la inspiración. Es un trastorno que afecta principalmente a perros ancianos de razas grandes a gigantes y con etiología congénita o adquirida, afectando especialmente a razas, especialmente a razas como Bouvier des Flandres, Siberian Huskie, Bull Terrier, Pastor Alemán, Labrador, Golden Retriever, entre otras. Los signos clínicos incluyen obstrucción de las vías aéreas anteriores durante la inspiración, intolerancia al ejercicio, estridor inspiratorio y cambios en la vocalización, generando un ladrido débil y ronco, además de ahogo y tos esporádica. El diagnóstico comienza con los signos clínicos, el examen físico y es definitivo con la inspección de la función laríngea por laringoscopia directa o con la ayuda de un endoscopio. El tratamiento de soporte consiste en estabilizar al paciente con oxigenoterapia y administrar fármacos antiinflamatorios. En animales con dificultad respiratoria, se debe instituir una terapia de emergencia. El tratamiento quirúrgico es de elección y consiste en estabilizar y reposicionar los cartílagos aritenoides. Este trabajo tiene como objetivo reportar un caso clínico de un canino afectado con parálisis laríngea y describir la investigación diagnóstica por laringoscopia directa.A laringe é o órgão que permite a passagem de ar para o trato respiratório caudal através do controle do fluxo de ar durante o processo de respiração. A paralisia de laringe consiste na incapacidade uni ou bilateral das cartilagens aritenoides em abduzirem durante a inspiração. É um distúrbio que afeta principalmente cães idosos de raças grandes a gigantes e com etiologia congênita ou adquirida acometendo especialmente raças como Bouvier des Flandres, Huskie Siberiano, Bull Terrier, Pastor Alemão, Labrador, Golden Retriever, dentre outros. Os sinais clínicos incluem obstrução das vias respiratórias anteriores na inspiração, intolerância ao exercício, estridor inspiratório e alteração na vocalização, gerando um latido fraco e rouco, além de engasgos e tosses esporádicas. O diagnóstico inicia-se com os sinais clínicos, exame físico, e é definitivo pela inspeção da função laríngea por laringoscopia direta ou com auxílio de endoscópio. O tratamento de suporte consiste na estabilização do paciente com oxigenoterapia e administração de anti-inflamatórios. Em animais com angústia respiratória, terapêutica emergencial deve ser instituída. O tratamento cirúrgico é o de eleição e consiste na estabilização e reposicionamento das cartilagens aritenoides.
