Nursing care for newborns with perinatal asphyxia undergoing therapeutic hypothermia: an integrative literature review

Resumo: | Perinatal asphyxia is defined as na injury to the newborn, characterized by oxygen deprivation (hypoxia) and perfusion disorder, which can produce multiple systemic repercussions. Hypoxic-ischemic encephalopathy is the most serious consequence of perinatal asphyxia and is among the main causes of perinatal mortality and morbidity. Currently, therapeutic hypothermia, also called induced, presents itself as a neuroprotective therapy capable of reducing mortality and improving the neurological outcome in this clientele, however the success of therapy requires the training of nurses, in order to provide qualified and safe care. This study aims to describe, according to the literature, the nursing care provided to the newborn submitted to therapeutic hypothermia. It is na integrative review carried out between September and October 2020, using electronic sources from the Virtual Health Library, PubMed, CINAHL, SCIELO, Web of Science and SCOPUS, with a time frame from 2015 to 2020. 1330 publications were found, of which 16 articles were included in the review according to the eligibility criteria established. Essential care: maintenance of central body temperature, hemodynamic monitoring, glycemic control, surveillance of the Integrated Amplitude Electroencephalogram, observation of the skin and communication with the ultid. Therapy has benefits, but its application depends on countless care involving the ultidisciplinar team. In addition, the breadth and relevance of the nurse’s performance during therapy became notorious, as nursing care is essential for maintaining treatment and preventing possible complications and adverse effects of therapeutic hypothermia. |

La encefalopatía hipóxico-isquémica es la consecuencia más grave de la asfixia perinatal y se encuentra entre las principales causas de mortalidad y morbilidad perinatal. Actualmente, la hipotermia terapéutica, también llamada inducida, se presenta como una terapia neuroprotectora capaz de reducir la mortalidad y mejorar el resultado neurológico en esta clientela, sin embargo, el éxito de la terapia requiere la formación de enfermeras, con el fin de brindar una atención calificada y segura. Este estudio tiene como objetivo describir, de acuerdo con la literatura, los cuidados de enfermería brindados al recién nacido sometido a hipotermia terapéutica. Se trata de una revisión integradora realizada entre septiembre y octubre de 2020, utilizando fuentes electrónicas de la Biblioteca Virtual en Salud, PubMed, CINAHL, SCIELO, Web of Science y SCOPUS, con un marco temporal de 2015 a 2020. Se encontraron 1330 publicaciones, de las cuales 16 artículos fueron incluidos en la revisión según los criterios de elegibilidad establecidos. Cuidados esenciales: mantenimiento de la temperatura corporal central, monitorización hemodinámica, control glucémico, vigilancia del Electroencefalograma Integrado de Amplitud, observación de la piel y comunicación con la familia. La terapia tiene beneficios, pero su aplicación depende de innumerables cuidados que involucran al equipo multidisciplinario. Además, se hizo notoria la amplitud y relevancia del desempeño del enfermero durante la terapia, ya que el cuidado de enfermería es fundamental para mantener el tratamiento y prevenir posibles complicaciones y efectos adversos de la hipotermia terapêutica.A asfixia perinatal é definida como um agravo ao recém-nascido, caracterizado por privação de oxigênio (hipóxia) e distúrbio perfusional, podendo produzir múltiplas repercussões sistêmicas. A encefalopatia hipóxico-isquêmica é a consequência mais grave da asfixia perinatal e encontra-se entre as principais causas de mortalidade e morbidade perinatal. Atualmente, a hipotermia terapêutica, também chamada de induzida, se apresenta como uma terapia neuroprotetora capaz de reduzir a mortalidade e melhorar o desfecho neurológico nessa clientela, porém o sucesso da terapia requer a capacitação dos enfermeiros, com a finalidade de prestar um cuidado qualificado e seguro. Esse estudo tem por objetivo descrever, segundo a literatura, os cuidados de enfermagem prestados ao recém-nascido submetido à hipotermia terapêutica. Trata-se de uma revisão integrativa realizada entre setembro e outubro de 2020, em fontes eletrônicas da Biblioteca Virtual de Saúde, PubMed, CINAHL, SCIELO, Web of Science e SCOPUS, com recorte temporal de 2015 a 2020. Encontrou-se 1330 publicações, das quais 16 artigos foram incluídos na revisão conforme os critérios de elegibilidade estabelecidos. São cuidados essenciais: manutenção da temperatura corporal central, monitorização hemodinâmica, controle glicêmico, vigilância do Eletroencefalograma de Amplitude Integrada, observação da pele e comunicação com a família. A terapêutica apresenta benefícios, porém sua aplicação depende de inúmeros cuidados envolvendo a equipe multidisciplinar.
