Factors associated with early weaning from breastfeeding: a review

Resumo: The present article aims to evaluate which are the factors from biopsychosocial basis and obstetrician aspects and assistance in health are associated with the practice of early weaning therefore being able to assist the behaviors on the subject and increase the breastfeeding index. It is a literature integrative review, descriptive research, qualitative approach and deductive method performed by the electronic data base PubMed, MEDLINE, LILACS, VHL, BDENF e SciELO, using the index terms “aleitamento materno/breatsfeeding” and “desmame/weaning” with the boolean operators “e/AND”. In the initial search there were found 4407 articles. After applying the criteria of eligibility and reading, 22 articles were analyzed. The exclusive breastfeeding rates fluctuate between 14,8% and 98,1%. The main factors that had association with early weaning were: older age of the child, maternal work, use of dairy compound in the hospital, use of pacifier, mom with depressive symptoms. In contrast, mothers with past experience, paternal support, medium and high levels of self-efficacy, being in EBF in the hospital discharge and receive professional support and incentive in prenatal, immediate postpartum and late puerperium were protection factors to breastfeeding. Breastfeeding is determinate by multiple factors. The variables of assistance basis are a central aspect when breastfeeding is analyzed and the focus on maternal perception is necessary for the establishment and success of breastfeeding through understanding of obstacles, uncertainly and dissatisfaction with the process.

Se trata de una revisión integradora de literatura, investigación descriptiva, abordaje cualitativo y método deductivo realizada por la base de datos electrónica PubMed, MEDLINE, LILACS, BVS, BDENF y SciELO, utilizando los términos índice “aleitamento materno/breatsfeeding” y “desmame/weaning ” con los operadores booleanos “e/AND”. En la búsqueda inicial se encontraron 4407 artículos. Después de aplicar los criterios de elegibilidad y lectura, se analizaron 22 artículos. Las tasas de lactancia materna exclusiva fluctúan entre 14,8% y 98,1%. Los principales factores que tuvieron asociación con el destete temprano fueron: mayor edad del niño, trabajo materno, uso de compuesto lácteo en el hospital, uso de chupete, mamá con síntomas depresivos. En contrapartida, las madres con experiencia previa, apoyo paterno, niveles de autoeficacia medio y alto, estar en LME al alta hospitalaria y recibir apoyo e incentivo profesional en el prenatal, posparto inmediato y puerperio tardío fueron factores de protección a la lactancia materna. La lactancia materna está determinada por múltiples factores. Las variables de base asistencial son un aspecto central cuando se analiza la lactancia materna y el enfoque en la percepción materna es necesario para el establecimiento y éxito de la lactancia materna a través de la comprensión de los obstáculos, la incertidumbre y la insatisfacción con el proceso.O presente artigo objetiva avaliar quais os fatores de caráter biopsicossociocultural e aspectos obstétricos e assistenciais na saúde estão associados a prática de desmame precoce do aleitamento materno a fim de auxiliar nas ações frente ao tema de modo a melhorar os índices da amamentação. Trata-se de uma revisão integrativa da literatura, pesquisa descritiva, abordagem qualitativa e método dedutivo realizada a partir das bases de dados eletrônicas PubMED, MEDLINE, LILACS, BVS, BDENF e SciELO, usando os descritores “aleitamento materno/breastfeeding” e “desmame/weaning” com os operadores booleanos “e/AND”. Na busca inicial, foram encontrados 4407 artigos. Após aplicação dos critérios de elegibilidade e leitura, foram analisados 22 trabalhos. As taxas de aleitamento materno exclusivo variaram entre 14,8% e 98,1%. Os principais fatores associados ao desmame precoce foram: maior idade da criança, trabalho materno, uso de complemento lácteo no hospital, uso de chupeta, mãe com sintomas depressivos. Em contrapartida, mães com experiência anterior, apoio paterno, níveis médios e elevados de autoeficácia, estar em AME na alta hospitalar e receber apoio e incentivo profissional no pré-natal, pós-parto imediato e puerpério tardio foram fatores de proteção ao aleitamento. A amamentação é determinada por múltiplos fatores.
