Clinical nursing and breastfeeding care in the Kangaroo Unit

Resumo: | To reflect on aspects of clinical nursing care for premature in the context of the kangaroo unit and its contribution to the prevention of early weaning. Integrative literature review, carried out from August to September 2019, in the following bases: VHL; PubMed via MedLine; Scielo and LILACS, using the search equation: premature newborn AND family relationships AND breastfeeding. The data were organized in a flowchart and table, later the studies were analyzed in a reflective way. 201 articles were found, but only 10 articles contemplated the search. The context of the Kangaroo Unit and its peculiarities allow the Nurse to understand the bond between the mother-child binomial, broken with premature birth and hospitalization. Among the difficulties that arise in the Kangaroo Unit is breastfeeding, because depending on the length of stay, the mother may have reduced milk production, not meeting the baby's needs. In the Kangaroo Unit, the support network is not just family, but institutional through the health team that serves the dyad. Perceiving the nursing team as a support network for care and encouraging the practice of breastfeeding becomes the key point to increase breastfeeding rates among premature infants. |

Revisión integrativa de la literatura, realizada de agosto a septiembre de 2019, en las siguientes bases: BVS; PubMed a través de MedLine; Scielo y LILACS, utilizando la ecuación de búsqueda: recién nacido prematuro Y relaciones familiares Y lactancia materna. Los datos fueron organizados en un diagrama de flujo y tabla, posteriormente los estudios fueron analizados de manera reflexiva. Se encontraron 201 artículos, pero solo 10 artículos contemplaron la búsqueda. El contexto de la Unidad Canguro y sus peculiaridades permiten al Enfermero comprender el vínculo entre el binomio madre-hijo, roto con el parto prematuro y la hospitalización. Entre las dificultades que se presentan en la Unidad Canguro está la lactancia materna, pues dependiendo del tiempo de estadía, la madre puede tener reducida la producción de leche, no satisfaciendo las necesidades del bebé. En la Unidad Canguro, la red de apoyo no es solo familiar, sino institucional a través del equipo de salud que atiende a la díada. Percibir al equipo de enfermería como una red de apoyo para el cuidado y fomentar la práctica de la lactancia materna se convierte en el punto clave para aumentar las tasas de lactancia materna entre los prematuros.Refletir sobre os aspectos do cuidado clínico de enfermagem ao prematuro no contexto da unidade canguru e sua contribuição na prevenção do desmame precoce. Revisão integrativa da literatura, realizada de agosto a setembro de 2019, nas bases: BVS; PubMed via MedLine; Scielo e LILACS, utilizando a equação de busca: recém-nascido prematuro AND relações familiares AND aleitamento materno. Os dados foram organizados em fluxograma e quadro, posteriormente analisou-se de forma reflexiva os estudos. Foram encontrados 201 artigos, mas apenas 10 artigos contemplaram a pesquisa. O contexto da Unidade Canguru e suas peculiaridades proporcionam ao Enfermeiro compreender o vínculo entre o binômio mãe-filho, quebrado com o nascimento prematuro e a internação. Dentre as dificuldades que surgem na Unidade Canguru está a amamentação, pois dependendo do tempo de internação a mãe pode ter redução da produção láctea, não suprindo as necessidades do bebê. Na Unidade Canguru a rede de apoio não é apenas familiar, mas sim institucional através da equipe de saúde que atende a díade.
