Angle Class II treatment using the PowerScope II mandibular propeller: Clinical case report

Resumo: Angle Class II is one of the malocclusions most sought for treatment and a great challenge for the orthodontists. When patients present a mandibular deficiency still in the growth phase, functional appliances are usually chosen to correct this problem. However, in some situations, the lack of patient cooperation in using these appliances, especially removable ones, may compromise the result of the orthodontic treatment. Considering the evolution of mandibular propellers, several function models started to appear in the market, and among them is the PowerScope - a fixed hybrid propeller. Nevertheless, one of the effects expected from all propellers, either removable or fixed ones, besides dentoalveolar remodeling, is an excessive labial inclination of lower incisors. Thus, this study aimed to show the use of the PowerScope II for correcting Angle Class II in a 12-year-old patient with mandibular deficiency, brachyfacial, and with proclination of upper and lower incisors. After detailed planning, a 0.022" Roth prescription fixed appliance was selected, followed by the PowerScope II and lower bilateral miniscrews for anchorage, to contain the excessive proclination of lower incisors. At the end of treatment, the patient maintained the convex profile with the malocclusion correction by dentoalveolar compensation, achieving a satisfactory control of the inclination of lower incisors. Thus, it may be concluded that using this appliance associated with miniscrews for anchorage allows a conservative approach in Class II treatments.

Cuando un paciente se presenta con deficiencia mandibular y aún está en fase de crecimiento, los aparatos funcionales, generalmente, se eligen para la corrección de este problema. No obstante, en algunas situaciones, la falta de colaboración del paciente para usar estos aparatos, especialmente los removibles, puede comprometer el resultado del tratamiento ortodóncico. Con la evolución de los aparatos propulsores mandibulares, varios modelos funcionales comenzaron a surgir en el mercado y, entre ellos, el PowerScope - un propulsor fijo híbrido. Sin embargo, uno de los efectos esperados de todos los propulsores, sean estos removibles o fijos, además de la remodelación dentoalveolar, es la inclinación labial excesiva de los incisivos inferiores. De esta forma, el objetivo del presente estudio fue demostrar el uso del PowerScope II, para la corrección de Clase II de Angle em un paciente de 12 años con deficiencia mandibular, braquifacial y con incisivos superiores e inferiores vestibularizados. Tras planeamiento detallado, se optó por utilizar aparatología fija con prescripción Roth 0,022” seguida por el PowerScope II y minitornillos inferiores bilaterales, como forma de anclaje, para contener la vestibularización excesiva de los incisivos inferiores. Al final del tratamiento el paciente mantuvo el perfil convexo con corrección de la maloclusión por la compensación dentoalveolar y obtuvo un buen control de inclinación de los incisivos inferiores. Así, se puede concluir que la utilización de este dispositivo asociado a los minitonillos de anclaje permiten un abordaje conservador en tratamientos de Clase II.A Classe II de Angle, além de ser uma das más oclusões mais procuradas para tratamento, é também um grande desafio para os ortodontistas. Quando um paciente se apresenta com deficiência mandibular e ainda em fase de crescimento, os aparelhos funcionais, geralmente, são escolhidos para a correção deste problema. Porém, em algumas situações, a falta de colaboração do paciente no uso destes aparelhos, especialmente os removíveis, podem comprometer o resultado do tratamento ortodôntico. Com a evolução dos aparelhos propulsores mandibulares, vários modelos funcionais começaram a surgir no mercado e, dentre estes, o PowerScope, um propulsor fixo híbrido. Entretanto, um dos efeitos esperados de todos os propulsores, sejam eles removíveis ou fixos, além da remodelação dento-alveolar, é a inclinação labial excessiva dos incisivos inferiores. Desta forma, o objetivo desse estudo foi demonstrar o uso do PowerScope II, na correção de Classe II de Angle em um paciente de 12 anos com deficiência mandibular, braquifacial e com incisivos superiores e inferiores vestibularizados. Após planejamento detalhado, optou-se por utilizar aparatologia fixa com prescrição Roth 0,022”, seguida pelo PowerScope II e mini parafusos inferiores bilaterais, como forma de ancoragem, para conter a vestibularização excessiva dos incisivos inferiores. Ao fim do tratamento o paciente manteve o perfil convexo com correção da má oclusão pela compensação dentoalveolar e conseguiu um bom controle de inclinação dos incisivos inferiores.
