Thrombotic effects on users of combined oral contraceptives throughout their fertile life: integrative review

Resumo: Combined hormonal contraceptives (CHC) are used among women of the wordwide, increasing the risk of venous thromboembolism (VTE) including deep vein thrombosis (DVT) and pulmonary embolism (PE). The objective was to identify studs with thrombotic effects in women of child-bearing age associated with the use of combined oral contraceptives. The methodology used was an integrative review of the PubMed database articles using the health descriptors: “combined oral contraceptives”, “thrombotic effects”,  “blood clot”, and Portuguese version. The result found in the article showed that the continuous and prolonged use of CHC increase the risk of platelet aggregation and low thrombolysis favoring the formation of thrombi, resulting in accentuated DVT developtment. Thus, it is important to observe the medical eligibility criteria to define use of CHC for patients taking into account the risks a thrombotic disorder.

El objetivo fue identificar estudios en mujeres en edad fértil con efectos trombóticos asociados al uso de anticonceptivos orales combinados. La metodología utilizada fue una revisión integradora de los artículos de la base de datos PubMed utilizando los descriptores: “anticonceptivos orales combinados”, “efectos trombóticos”, “coágulo de sangre” y las versiones en inglés. Los resultados encontrados en los artículos demostraron que el uso continuo y prolongado de HCC aumenta el riesgo de agregación plaquetaria y baja trombolisis, favoreciendo la formación de trombos, con el consecuente desarrollo marcado de TEV. Por lo tanto, es importante observar los criterios médicos de elegibilidad para definir el uso de HCC para los pacientes, teniendo en cuenta los riesgos de un trastorno trombótico.Os contraceptivos hormonais orais combinados (CHC) são utilizados entre mulheres no mundo, elevando o risco de tromboembolismo venoso (TEV), incluindo trombose venosa profunda (TVP) e embolia pulmonar (EP). O objetivo foi identificar estudos com efeitos trombóticos em mulheres na idade fértil associados ao uso de contraceptivos orais combinados. A metodologia utilizada foi revisão integrativa dos artigos da base de dados PubMed utilizando os descritores: “anticoncepcionais orais combinados”, "efeitos trombóticos", "coágulo sanguíneo”, e as versões em inglês. Os resultados encontrados nos artigos demonstraram que o uso contínuo e prolongado de CHC aumentam o risco de agregação plaquetária e baixa trombólise favorecendo a formação de trombos, tendo como consequência desenvolvimento acentuado de TEV.
