Evaluation of histopathological aspects in squamous cell carcinomas: an integrative literature review

Resumo: Introduction: Squamous cell carcinoma represents the most frequent malignant neoplasm in the oral cavity, and its incidence is more recurrent in patients who smoke and/or drink alcohol, being even more evident when they are male and of advanced age. In addition, histopathological analysis is essential for the detection of oral malignancy. Methodology: The following research question was developed: What are the main histopathological characteristics for the diagnosis of squamous cell carcinomas? For this, the electronic database: U. S. National Library of Medicine (PubMed) was used to search and identify studies that answered the guiding question of this integrative literature review. The database was searched for studies carried out between January 1990 and April 2022. Two descriptors were used to compose the search key, the following (MeSH): “Squamous cell carcinoma”; “Histopathological”; "Dentistry". Then, the researchers selected the works with analysis in the title and abstract, based on the eligibility criteria. Eligibility criteria were as follows: articles published in English, Portuguese and Spanish; publications between January 2000 and April 2022; human research; articles that fit the theme. The advanced form system was also used to search and select articles using the Boolean “AND” connector. Then, articles that met the eligibility criteria were identified and included in the review. Results and discussion: Based on this search strategy, a total of 521 works were found in full; of these, 4 articles were duplicated in the search strategies, thus totaling 10 selected. The histological characteristics varied in each case, and some tumor infiltrations, atypical mitotic proliferations and subcutaneous infiltrations of lymphatic cells could be found. Final considerations: The histopathological examination is an important tool in the prior identification of SCC, helping in the clinical detection before the development of the neoplasm. Allied to this, it is necessary for the Dental Surgeon to pay close attention to potentially malignant lesions, such as leukoplakia and erythroplakia, since most oral cancers are diagnosed late, which impairs therapy and facilitates the spread of tumor cells.

Además, el análisis histopatológico es fundamental para la detección de malignidad oral. Metodología: Se desarrolló la siguiente pregunta de investigación: ¿Cuáles son las principales características histopatológicas para el diagnóstico de los carcinomas de células escamosas? Para ello, se utilizó la base de datos electrónica: U. S. National Library of Medicine (PubMed) para buscar e identificar estudios que respondieran a la pregunta guía de esta revisión integradora de literatura. Se realizaron búsquedas en la base de datos de estudios realizados entre enero de 1990 y abril de 2022. Se utilizaron dos descriptores para componer la clave de búsqueda, los siguientes (MeSH): “Carcinoma de células escamosas”; “Histopatológico”; "Odontología". Luego, los investigadores seleccionaron los artículos con análisis en el título y el resumen, con base en los criterios de elegibilidad. Los criterios de elegibilidad fueron los siguientes: artículos publicados en inglés, portugués y español; publicaciones entre enero de 2000 y abril de 2022; investigación humana; artículos que se ajusten al tema. El sistema de formulario avanzado también se utilizó para buscar y seleccionar artículos utilizando el conector booleano "Y". Luego, los artículos que cumplieron con los criterios de elegibilidad fueron identificados e incluidos en la revisión. Resultados y discusión: Con base en esta estrategia de búsqueda, se encontraron un total de 521 trabajos completos; de estos, 4 artículos fueron duplicados en las estrategias de búsqueda, totalizando así 10 seleccionados. Las características histológicas variaron en cada caso, pudiendo encontrarse algunas infiltraciones tumorales, proliferaciones mitóticas atípicas e infiltraciones subcutáneas de células linfáticas. Consideraciones finales: El examen histopatológico es una herramienta importante en la identificación previa de SCC, ayudando en la detección clínica antes del desarrollo de la neoplasia. Unido a esto, es necesario que el Cirujano Dentista preste mucha atención a las lesiones potencialmente malignas, como la leucoplasia y la eritroplasia, ya que la mayoría de los cánceres orales se diagnostican tardíamente, lo que dificulta la terapia y facilita la diseminación de las células tumorales.Introdução: O carcinoma espinocelular representa a neoplasia maligna mais frequente na cavidade oral, e sua incidência é mais recorrente em pacientes tabagistas e/ou etilistas, mostrando-se ainda mais evidente quando estes são do sexo masculino e de idade avançada. Além disso, a análise histopatológica é imprescindível à detecção da neoplasia maligna bucal. Metodologia: Desenvolveu-se a seguinte pergunta de pesquisa: Quais são as principais características histopatológicas para o diagnóstico dos carcinomas espinocelulares? Para isto, foi utilizada a base de dados eletrônica: U. S. National Library of Medicine (PubMed) para pesquisar e identificar estudos que respondessem à pergunta norteadora desta revisão integrativa da literatura. A base de dados foi pesquisada para estudos realizados entre janeiro de 1990 a abril de 2022. Foram utilizados dois descritores para a composição da chave de pesquisa, sendo os seguintes (MeSH): “Squamous cell carcinoma”; “Histopathological”; “Dentistry”. Em seguida, os pesquisadores selecionaram os trabalhos com análise no título e resumo, com base nos critérios de elegibilidade. Os critérios de elegibilidade foram os seguintes: artigos publicados em inglês, português e espanhol; publicações entre janeiro de 2000 e abril de 2022; pesquisa em humanos; artigos que se adequem à temática. Também foi utilizado o sistema de formulário avançado para busca e seleção dos artigos utilizando conector booleano “AND”. Em seguida, artigos que preencheram os critérios de elegibilidade foram identificados e incluídos na revisão. Resultados e discussão: A partir dessa estratégia de busca, foram encontrados ao todo 521 trabalhos na íntegra; destes, 4 artigos encontravam-se duplicados nas estratégias de busca, totalizando, assim, 10 selecionados. As características histológicas foram variadas a cada caso, podendo ser encontradas algumas infiltrações tumorais, proliferações mitóticas atípicas e infiltrações subcutâneas de células linfáticas. Considerações finais: O exame histopatológico é uma ferramenta importante na identificação prévia do CEC, auxiliando na detecção clínica antes do desenvolvimento da neoplasia.
