Causes of removal of the central catheter for peripheral insertion in newborns in a School Hospital in Southern Brazil

Resumo: | To analyze the use and causes of early removal of the peripherally inserted central catheter in newborns and to develop strategies in the future for better use and maintenance of these catheters, providing more quality care for newborns admitted to the Neonatal Intensive Care Unit of a hospital university in southern Brazil. A cross-sectional retrospective quantitative study carried out through the review of peripherally inserted central catheter follow-up forms, from June 2014 to December 2018. A total of 216 newborns used peripherally inserted central catheter, 84% of which were premature. The main reasons for withdrawal were due to the end of treatment (37%), followed by obstruction (22%), breakage (17%), phlebitis (11%), extrusion (9%) and death (4%), respectively. In 63% of the newborns, it was found that the removal occurred early, with 8 days (interquartile range: 5; 12) after catheter insertion. Difficulties in maintaining the catheter were also found in 56% of cases. It was verified that the early exit of the catheter occurred more frequently when 4 antibiotics were used or more 76.3% (p<0.001) and when other drugs were used 66.3% (p = 0.038). It is concluded from the present study that the peripherally inserted central catheter is an important resource in prolonged intravenous therapy in the care of newborns and that early removal happens with high frequency, with the identification of some factors that should be avoided. This type of catheter must be monitored by professionals duly trained both for its insertion and for its maintenance. |

Estudio cuantitativo retrospectivo de corte transversal realizado a través de la revisión de fichas de seguimiento de catéter central de inserción periférica, desde junio de 2014 hasta diciembre de 2018. Un total de 216 recién nacidos utilizaron catéter central de inserción periférica, de los cuales el 84% fueron prematuros. Los principales motivos de abandono se debieron a la finalización del tratamiento (37 %), seguido de obstrucción (22 %), rotura (17 %), flebitis (11 %), extrusión (9 %) y muerte (4 %), respectivamente. En el 63% de los recién nacidos se encontró que la remoción ocurrió de manera temprana, con 8 días (rango intercuartílico: 5; 12) después de la inserción del catéter. También se encontraron dificultades en el mantenimiento del catéter en el 56% de los casos. Se encontró que el retiro temprano del catéter ocurrió con mayor frecuencia cuando se utilizaron 4 antibióticos o más 76,3% (p<0,001) y cuando se utilizaron otros fármacos 66,3% (p = 0,038). Se concluye del presente estudio que el catéter central de inserción periférica es un recurso importante en la terapia endovenosa prolongada en el cuidado de los recién nacidos y que la extracción temprana ocurre con alta frecuencia, con la identificación de algunos factores que deben ser evitados. Este tipo de catéter debe ser monitoreado por profesionales debidamente capacitados tanto para su inserción como para su mantenimiento.Analisar o uso e as causas das remoções precoce do cateter central de inserção periférica em recém-nascidos e para futuramente elaborar estratégias visando melhor utilização e manutenção destes cateteres proporcionando mais qualidade na assistência dos recém-nascidos internados na Unidade de Terapia Intensiva Neonatal de um hospital universitário no Sul do Brasil. Estudo quantitativo retrospectivo transversal realizado através da revisão de formulários de acompanhamento do cateter central de inserção periférica, no período de junho de 2014 a dezembro de 2018. Utilizaram cateter central de inserção periférica 216 recém-nascidos, sendo que 84% eram prematuros. Os principais motivos de retirada foram ocasionados pelo término do tratamento (37%), seguidos de obstrução (22%), quebra (17%), flebite (11%), extrusão (9%) e óbito (4%), respectivamente. Em 63% dos recém-nascidos constatou-se que a retirada ocorreu precocemente, com 8 dias (Intervalo interquartil: 5; 12) após a inserção do cateter. Foram encontradas ainda dificuldades para a manutenção do cateter em 56% dos casos. Foi verificado que a saída precoce do cateter ocorreu com maior frequência quando eram utilizados 4 antibióticos ou mais 76,3% (p<0,001) e quando eram utilizadas outras drogas 66,3% (p = 0,038). Conclui-se através do presente estudo que o cateter central de inserção periférica é um recurso importante na terapia intravenosa prolongada no cuidado aos recém-nascidos e que a retirada precoce acontece com elevada frequência, com a identificação de alguns fatores que devem ser evitados.
