The efficiency and safety in the implementation of physical exercise protocols as a non-drug treatment for chronic renal patients on hemodialysis and the difficulties of implementation in clinical practice: an integrative review

Resumo: Chronic kidney disease (CKD) is a progressive kidney injury with a decreased glomerular filtration rate for more than three months. in its more advanced stage, it is called end-stage chronic renal failure. At this stage, patients are indicated for renal replacement therapy (RRT) or kidney transplantation. CKD brings with it several comorbidities such as anemia, chronic inflammatory state, hypertension, diabetes, chronic uremia. The anti-inflammatory role of regular physical exercise is closely correlated with the decrease in the progression of CKD and also with the patients' prognosis. This study aims to establish the effects and safety of regular physical exercise on the prognosis of patients with chronic kidney disease. This is an integrative review that used the consultation in the SCIELO, PUBMED and LILACS electronic databases, in September 2021, using the following previous criteria: publication date from January 1, 2016 to September 30, 2021; English language; descriptors included in the title and/or abstract – exercise (exercise), hemodialysis (haemodialysis) and chronic kidney disease (chronic kidney disease), free text, and only randomized clinical trials should be selected. The results showed a significant improvement in the modulation of inflammatory biomarkers, Kt/V, muscle strength, cardiorespiratory reserve, sleep pattern and health-promoting behaviors. Based on the results of this study, the intradialytic PE program proved to be efficient and feasible to apply. Even low-intensity exercise, even passively, had positive impacts on the quality of life and health of these individuals.

Su progresión se denomina insuficiencia renal crónica terminal. En esta etapa, los pacientes están indicados para terapia de reemplazo renal (TRS) o trasplante de riñón. La ERC se relaciona con anemia, estado inflamatorio crónico, hipertensión, diabetes, uremia crónica. El papel antiinflamatorio del ejercicio físico está estrechamente relacionado con la disminución de la progresión de la ERC y también con el pronóstico de los pacientes. Este estudio tiene como objetivo establecer los efectos y la seguridad del ejercicio físico regular en el pronóstico de los pacientes con enfermedad renal crónica. Se trata de una revisión integradora que utilizó la consulta en las bases de datos electrónicas SCIELO, PUBMED y LILACS, en septiembre de 2021, utilizando los siguientes criterios previos: fecha de publicación entre enero de 2016 y septiembre de 2021; Idioma Inglés; Se deben seleccionar descriptores incluidos en el título y / o resumen: ejercicio (ejercicio), hemodiálisis (hemodiálisis) y enfermedad renal crónica (enfermedad renal crónica), texto libre y solo ensayos clínicos aleatorizados. Los resultados mostraron una mejora significativa en los biomarcadores inflamatorios, Kt / V, fuerza muscular, reserva cardiorrespiratoria, patrón de sueño y comportamientos. Con base en los resultados de este estudio, el programa de EF intradialítica demostró ser eficiente y factible. Incluso el ejercicio de baja intensidad, incluso de forma pasiva, tuvo un impacto positivo en la calidad de vida y la salud de estas personas.A doença renal crônica (DRC) consiste em lesão renal progressiva com taxa de filtração glomerular diminuída por mais de três meses. em sua fase mais avançada, é denominada de insuficiência renal crônica em fase terminal. Nesta fase, os pacientes têm indicação de terapia renal substitutiva (TRS) ou transplante renal. A DRC traz consigo várias comorbidades como a anemia, estado inflamatório crônico, hipertensão, diabetes, uremia crônica. O papel anti-inflamatório da prática regular de exercícios físicos tem correlação íntima com a diminuição na progressão da DRC e também com o prognóstico dos pacientes. Esse estudo visa estabelecer os efeitos e a segurança da prática regular de exercícios físicos no prognóstico de pacientes com doença renal crônica. Trata-se de uma revisão integrativa que utilizou a consulta nas bases de dados eletrônicas SCIELO, PUBMED e LILACS,em setembro de 2021, utilizando os seguintes critérios prévios: data de publicação de 01 de janeiro de 2016 a 30 de setembro de 2021; idioma inglês; descritores incluídos no título e/ou resumo – exercício (exercise), hemodiálise (haemodialysis) e doença renal crônica (chronic kidney disease), texto gratuito, devendo serem selecionados apenas ensaios clínicos randomizados. Os resultados evidenciaram uma melhora significativa na modulação de biomarcadores inflamatórios, Kt/V, força muscular, reserva cardiorrespiratória, padrão de sono e comportamentos promotores de saúde. Com base nos resultados deste estudo, o programa de EF intradialítico mostrou-se eficiente e de aplicabilidade factível.
