Outcomes of post-bariatric pregnancy in the maternal-fetal binomial: an integrative review

Resumo: Obesity is a high prevalence condition worldwide, and bariatric surgery is the most effective long-term treatment. Half these surgeries are performed in women of potentially fertile ages, but it is yet not clear what are the outcomes of having a pregnancy in this postoperative period. Accordingly, an integrative review was performed to approach maternal obesity and their implications on the health of the maternal-fetal binomial. Accordingly, an integrative review was performed on Pubmed from march to May 2021 to raise data about Post-bariatric pregnancy. 424 articles dated between 2010 and 2020 were found using the keywords “Pregnancy”, “Bariatric Surgery” and “outcome”. 13 publications dated between 2010 and 2020 were analyzed. The year 2020 had the highest publication prevalence, with a total of 4 (30,79%) articles. Considering the different techniques used to perform bariatric surgery, it was concluded that the malabsorptive procedure has the highest chance of complicating, especially by having a small gestational age fetus. Furthermore, assessing the interval between surgery and conception, no consensus was found. Therefore, there is need for multi-professional orientation for post-bariatric pregnant women and adequate vitamin supplementation, which is related to better pregnancy outcomes and lower rates of fetal malformation.

La mitad de estas cirugías se realizan en mujeres en edad potencialmente fértil, pero aún no está claro cuáles son los resultados de tener un embarazo en este postoperatorio. En consecuencia, se realizó un estudio integrativo para abordar la obesidad materna y sus implicaciones en la salud del binomio materno-fetal. En consecuencia, se realizó un estudio integrativo en Pubmed desde marzo hasta mayo de 2021 para recopilar datos sobre el embarazo posbariátrico. Se encontraron 424 artículos fechados entre 2010 y 2020 utilizando las palabras clave “Embarazo”, “Cirugía bariátrica” y “resultado”. Se analizaron 13 publicaciones fechadas entre 2010 y 2020. El año 2020 tuvo la mayor prevalencia de publicación, con un total de 4 (30,79%) artículos. Teniendo en cuenta las diferentes técnicas utilizadas para realizar la cirugía bariátrica, se concluyó que el procedimiento malabsortivo tiene la mayor probabilidad de complicarse, especialmente por tener un feto de edad gestacional pequeña. Además, al evaluar el intervalo entre la cirugía y la concepción, no se encontró consenso. Por lo tanto, existe la necesidad de una orientación multiprofesional para las mujeres embarazadas posbariátricas y una adecuada suplementación vitamínica, lo que se relaciona con mejores resultados del embarazo y menores tasas de malformación fetal.A obesidade é uma condição de alta prevalência em todo o mundo, e a cirurgia bariátrica é o tratamento mais eficaz a longo prazo. Nesse sentido, foi realizado um estudo integrativo para abordar a obesidade materna e suas implicações na saúde do binômio materno-fetal. Além disso, avaliando o intervalo entre a cirurgia e a concepção, não foi encontrado consenso.
