Meanings of brain-dead patient care for intensive care unit nurses

Resumo: The objective of this study was to identify the meanings of care for patients with brain death from the perspective of nurses in an adult Intensive Care Unit. Methodology: Qualitative, descriptive, exploratory study, with 35 nurses working in intensive and private care in the Southeast regions of Brazil, carried out from March to September 2020. and transported to the Software Interface de R pourles Analyses Multidimensionnelles de Textes et de Questionnaires. Results: Two categories emerged: Brain death protocol directing care and experiencing care for brain-dead patients. The diagnosis of brain death was described as something normal, which follows a safe and systematized protocol, aimed at nursing care in the possibility of organ donation. Nurses considered the care of patients with brain death as a sad event, mainly because it is an irreversible situation, and because they witness the suffering and pain of family members. Final considerations: It was identified that the care of the patient with brain death is surrounded by feelings, emotions and opinions, which can contribute or diverge to the diagnosis, which reinforces the need to reflect and break the paradigms related to the theme.

Metodología: Estudio cualitativo, descriptivo, exploratorio con 35 enfermeros que actúan en unidades de cuidados intensivos de adultos de hospitales públicos y privados de las regiones Sudeste y Sur de Brasil, realizado de marzo a septiembre de 2020. Las entrevistas fueron audiograbadas, posteriormente transcritas y transportadas para el Software Interface de R pourles Analisis Multidimensionnelles de Textes et de Questionnaires. Resultados: Emergieron dos categorías: Protocolo de muerte cerebral dirigiendo el cuidado y experimentando el cuidado de pacientes con muerte cerebral. El diagnóstico de muerte encefálica fue descrito como algo normal, que sigue un protocolo seguro y sistematizado, orientado al cuidado de enfermería ante la posibilidad de donación de órganos. Los enfermeros consideraron el cuidado de pacientes con muerte encefálica como un evento triste, principalmente por tratarse de una situación irreversible, y por ser testigos del sufrimiento y dolor de los familiares. Consideraciones finales: Se identificó que el cuidado del paciente con muerte encefálica está rodeado de sentimientos, emociones y opiniones, que pueden contribuir o divergir al diagnóstico, lo que refuerza la necesidad de reflexionar y romper los paradigmas relacionados con la tema.O objetivo deste estudo foi identificar os significados do cuidado ao paciente em morte encefálica na perspectiva de enfermeiros de Unidade de Terapia Intensiva adulto. Metodologia: Estudo qualitativo, descritivo, exploratório, com 35 enfermeiros atuantes em unidades de terapia intensiva adulto de hospitais públicos e privados das Regiões Sudeste e Sul do Brasil, realizado de março a setembro de 2020. As entrevistas foram audiogravadas, e, posteriormente transcritas e transportadas para o Software Interface de R pourles Analyses Multidimensionnelles de Textes et de Questionnaires. Resultados: Emergiram duas categorias: Protocolo de morte encefálica direcionando o cuidado e vivenciando o cuidado ao paciente com morte cerebral. O diagnóstico de morte encefálica foi descrito como algo normal, que segue protocolo seguro e sistematizado, direcionado aos cuidados de enfermagem na possibilidade da doação de órgãos. Os enfermeiros consideraram o cuidado ao paciente em morte encefálica como um evento triste, principalmente por ser uma situação irreversível, e por presenciar o sofrimento e a dor dos familiares.
