Profile resistance of Escherichia coli in urocultures in 2020 in Cascavel/PR

Resumo: | Introduction: urinary tract infection (UTI) is one of the most frequent in the population, second only to those that affect the respiratory system. This pathology affects both sexes, with a prevalence in females. The main cause of UTIs is uropathogenic Escherichia coli (UPEC), representing up to 80% of infections. The patient's symptoms may vary according to the anatomical site of the infection. The gold standard diagnosis of UTI is the uroculture with antibiogram, due to its possibility to identify the microorganism that is causing the infection and guide antibiotictherapy. As the treatment is usually made empirically, there is an increasement of bacterial resistance to the antibiotics used, making requiring studies to analyze the resistance profile of bacteria. Methodology: for the execution of this retrospective study, results for the year 2020 of positive urine cultures for E. coli with antibiogram from patients with community-acquired UTI were collected from a clinical analysis laboratory in Cascavel/PR. Results: 358 adequate results were obtained for the inclusion criteria. Of these, 84.63% were female. The age group most affected was from 0 to 4 years old, representing 22.90% of cases. Of the antibiotics studied, nalidixic acid, ampicillin with sulbactam, ampicillin, aztreonam, cephalothin, cefepime, ciprofloxacin, levofloxacin, norfloxacin and trimethoprim with sulbactam were above the 20% resistance threshold. The other evaluated drugs showed resistance below 20%, making their practice viable. Conclusion: E. coli presented different prevalences and incidences between this and other studies, making its constant monitoring important. |

Esta patología afecta a ambos sexos, con predominio en el sexo femenino. La principal causa de las ITU es la Escherichia coli uropatógena (UPEC), que representa hasta el 80% de las infecciones. Los síntomas del paciente pueden variar según el sitio anatómico de la infección. El diagnóstico patrón oro de la ITU es el urocultivo con antibiograma, por su posibilidad de identificar el microorganismo que está causando la infección y orientar la antibioticoterapia. Como el tratamiento suele realizarse de forma empírica, se produce un aumento de la resistencia bacteriana a los antibióticos empleados, por lo que se requieren estudios para analizar el perfil de resistencia de las bacterias. Metodología: para la ejecución de este estudio retrospectivo, se recolectaron los resultados del año 2020 de urocultivos positivos para E. coli con antibiograma de pacientes con ITU adquirida en la comunidad de un laboratorio de análisis clínicos en Cascavel/PR. Resultados: se obtuvieron 358 resultados adecuados a los criterios de inclusión. De estos, el 84,63% eran mujeres. El grupo de edad más afectado fue el de 0 a 4 años, representando el 22,90% de los casos. De los antibióticos estudiados, el ácido nalidíxico, la ampicilina con sulbactam, la ampicilina, el aztreonam, la cefalotina, la cefepima, la ciprofloxacina, la levofloxacina, la norfloxacina y la trimetoprima con sulbactam superaron el umbral de resistencia del 20 %. Los demás fármacos evaluados presentaron resistencias por debajo del 20%, lo que viabiliza su práctica. Conclusión: E. coli presentó diferentes prevalencias e incidencias entre este y otros estudios, siendo importante su constante monitoreo.Introdução: a infecção do trato urinário (ITU) é uma das mais frequentes na população, atrás apenas daquelas que acometem o sistema respiratório. Esta patologia acomete ambos os sexos, havendo prevalência no sexo feminino. O principal causador das ITUs é a Escherichia coli uropatogência (UPEC), representando até 80% das infecções. A sintomatologia do paciente pode variar de acordo com o sítio anatômico da infecção. O diagnóstico padrão-ouro da ITU é a urocultura com antibiograma, tendo em vista sua possibilidade de identificar o microrganismo causador da infecção e guiar a antibioticoterapia. Como o tratamento costuma ser feito empiricamente, há um aumento da resistência bacteriana aos antibióticos utilizados, tornando necessário estudos que analisem o perfil de resistência das bactérias. Metodologia: para a execução desse estudo retrospectivo, foram coletados, a partir de um laboratório de análises clínicas de Cascavel/PR, resultados do ano de 2020 de uroculturas positivas para E. coli com antibiograma, provenientes de pacientes com ITU adquirida na comunidade. Resultados: foram obtidos 358 resultados adequados para os critérios de inclusão. Destes, 84,63% eram pertencentes ao sexo feminino. A faixa etária mais acometida foi de 0 a 4 anos, representando 22,90% dos casos. Dos antibióticos estudados, ficaram acima do limiar de 20% de resistência o ácido nalidixico, ampicilina com sulbactama, ampicilina, aztreonam, cefalotina, cefepima, ciprofloxacina, levofloxacina, norfloxacina e trimetoprima com sulbactama. Os demais medicamentos avaliados apresentaram resistência abaixo de 20%, tornando sua prática viável.
