Reestablishment of a canine guide with ceramic fragments in a bruxist patient: A case report

Resumo: | Introduction: Currently, there is a high rate of patients with signs and symptoms of temporomandibular disorders, of multifactorial etiology. Dental wear and clamping are the main complaints characterized by bruxism. In addition to affecting aesthetics, this change is responsible for modifying the patient's occlusion, impairing the relationship and distribution of forces between antagonistic teeth. It is known that, within the concept of mutually protected occlusion, the anterior teeth can receive oblique forces, while the posterior teeth receive the vertical ones. However, with the absence of a canine guide, during mandibular laterality movement, the posterior teeth present poorly distributed contacts, generating oblique masticatory forces, resulting in wear and interference, compromising the stomatognathic system. Objective: To describe procedures performed on a patient with a history of bruxism, returning a canine guide through the use of E-max ceramic fragments. Clinical case report: The absence of a canine guide was observed, whose treatment of choice was the reestablishment of the anatomical characteristics of the canines with fragments made in ceramics, occlusal adjustment, and a subsequent indication of continuous use of myorelaxant plaque in acrylic resin. Result in Functional treatment with satisfactory aesthetics and reestablishment of the patient's canine guide and its mutually protected occlusion. Conclusion: The reported treatment with ceramic fragments favored excursive mandibular movements and returned physiological aspects of the occlusion, contributing to the improvement of the signs and symptoms of bruxism and the patient's quality of life. |

El desgaste dental y el pinzamiento son las principales quejas caracterizadas por el bruxismo. Además de afectar la estética, este cambio es responsable de modificar la oclusión del paciente, perjudicando la relación y distribución de fuerzas entre dientes antagónicos. Se sabe que, dentro del concepto de oclusión mutuamente protegida, los dientes anteriores son capaces de recibir fuerzas oblicuas, mientras que los dientes posteriores reciben las verticales. Sin embargo, ante la ausencia de guía canina, durante el movimiento de lateralidad mandibular, los dientes posteriores presentan contactos mal distribuidos, generando fuerzas masticatorias oblicuas, resultando en desgaste e interferencia, comprometiendo el sistema estomatognático. Objetivo: Describir los procedimientos realizados a un paciente con antecedentes de bruxismo, devolviendo una guía canina mediante el uso de fragmentos cerámicos E-max. Reporte de caso clínico: Se observó ausencia de guía canino, cuyo tratamiento de elección fue el restablecimiento de las características anatómicas de los caninos con fragmentos realizados en cerámica, ajuste oclusal y posterior indicación de uso continuado de placa miorrelajante en resina acrílica. Resultado Tratamiento funcional con estética satisfactoria y restablecimiento de la guía canina del paciente y su oclusión mutuamente protegida. Conclusión: El tratamiento reportado con fragmentos cerámicos favoreció los movimientos mandibulares excursivos y devolvió los aspectos fisiológicos de la oclusión, contribuyendo a la mejora de los signos y síntomas del bruxismo ya la calidad de vida del paciente.Introdução: Atualmente é alto o índice de pacientes com sinais e sintomas de disfunções temporomandibulares, de etiologia multifatorial. Sendo o desgaste e apertamento dentário, as principais queixas caracterizadas pelo bruxismo. Além de afetar a estética, essa alteração é responsável pela modificação da oclusão do paciente, prejudicando a relação e distribuição de forças entre os dentes antagonistas. Sabe-se que, dentro do conceito de oclusão mutuamente protegida, os dentes anteriores estão aptos a receber forças oblíquas, enquanto os dentes posteriores recebem as verticais. Entretanto, com a ausência de guia canina, durante o movimento mandibular de lateralidade, os dentes posteriores apresentam contatos mal distribuídos, gerando forças oblíquas mastigatórias, resultando em desgastes e interferências comprometendo o sistema estomatognático. Objetivo: Descrever procedimentos executados em um paciente com histórico de bruxismo, devolvendo guia canina através da utilização de fragmentos cerâmicos E-max. Relato do caso clínico: Observou-se a ausência de guia canina, cujo tratamento de escolha foi o reestabelecimento das características anatômicas dos caninos com fragmentos confeccionados em cerâmica, ajuste oclusal e indicação posterior de uso contínuo de placa miorrelaxante em resina acrílica. Resultado Tratamento funcional com estética satisfatória e reestabelecimento da guia canina do paciente e sua oclusão mutuamente protegida.
