Influencing factors of non-adherence to treatment in patients diagnosed with hypertension

Resumo: Systemic arterial hypertension (SAH) is one of the main risk factors for cardiovascular mortality. Adherence to treatment is a challenge for public health. The objective of the study was to evaluate the main factors that lead to discontinuation of pharmacological treatment. We evaluated 100 hypertensive individuals of both sexes, aged over 30 years. They were divided into two groups; Group 1, composed of 77 individuals who adhered to pharmacological treatment, and Group 2, formed by 23 individuals who discontinued drug treatment for blood pressure control. The framingham cardiovascular score, individual profile questionnaire (NAHAS), morisky-green questionnaire, and sociodemographic questionnaire were applied in both groups. Chi-square and Fisher's exact test were used for significance analysis, and for statistical analysis, the statistical software EPI 6.0 and Graph Prima 5.0. It was found that the group of individuals who do not adhere to drug treatment present higher systolic and diastolic blood pressures than the group that adheres to treatment. It was possible to conclude that several factors have a direct influence on the adherence to pharmacological treatment and that lifestyle must be allied to medication.

La adherencia al tratamiento es un reto para la salud pública. El objetivo del estudio era evaluar los principales factores que conducen a la interrupción del tratamiento farmacológico. Se evaluó a 100 hipertensos de ambos sexos, mayores de 30 años. Se dividieron en dos grupos; el Grupo 1, compuesto por 77 individuos que se adhirieron al tratamiento farmacológico y el Grupo 2, formado por 23 individuos que interrumpieron el tratamiento farmacológico para el control de la presión arterial. En ambos grupos se aplicaron la puntuación cardiovascular de Framingham, el cuestionario de perfil individual (NAHAS), el cuestionario morisky-green y el cuestionario sociodemográfico. Para el análisis de significación se utilizaron la prueba de Chi-cuadrado y la prueba exacta de Fisher, y para el análisis estadístico, los programas informáticos estadísticos EPI 6.0 y Graph Prima 5.0. Se verificó que el grupo de individuos que no adhiere al tratamiento farmacológico presenta presiones arteriales sistólica y diastólica más elevadas que el grupo que adhiere al tratamiento. Se pudo concluir que varios factores influyen directamente en la adherencia al tratamiento farmacológico y que el estilo de vida debe ir unido a la medicación.A hipertensão Arterial Sistêmica (HAS) é um dos principais fatores de riscos de mortalidade cardiovascular. A adesão ao tratamento é um desafio para a saúde pública. O objetivo do estudo foi avaliar os principais fatores que levam à descontinuação do tratamento farmacológico. Foram avaliados 100 indivíduos de ambos os sexos, hipertensos e com idade superior a 30 anos. Eles foram divididos em dois grupos; Grupo 1, composto de 77 indivíduos que aderem ao tratamento farmacológico e o Grupo 2, formado por 23 indivíduos que descontinuaram o tratamento medicamentoso para controle da pressão arterial. Aplicaram-se em ambos o escore cardiovascular de framingham, questionário de perfil individual (NAHAS), questionário morisky-green e questionário sociodemográfico. Para a análise de significância foram utilizados os testes chi-quadrado e o teste exato de fisher e para análise estatística, os softwares estatísticos EPI 6.0 e Graph Prima 5.0. Verificou-se que o grupo de indivíduos que não aderem ao tratamento medicamentoso apresentam pressões sistólicas e diastólicas mais elevadas do que o grupo que adere ao tratamento.
