Management of Perforation of Schneider’s Membrane in Maxillary Sinus Lift with L-PRF - Case Report

Resumo: Hyperpneumatization of the maxillary sinus is a factor that complicates the insertion of implants in the posterior region of the maxilla. Prior maxillary sinus floor elevation is an alternative that will enable implant placement in these cases. However, intraoperative complications may occur, with the perforation of the Schneider membrane being the most common. Several maneuvers for the management of sinus membrane perforations are described in the literature, making it possible to repair and perform the grafting procedure during the same surgical time. Among the forms of repair, the use of leukocyte and platelet rich fibrin (L-PRF) membranes has been shown to be a treatment option with interesting and promising results to achieve repair. This article describes a clinical case in which the perforation of Schneider’s membrane was repaired with L-PRF membranes and the procedure was performed without postoperative complications, enabling the subsequent insertion of implants and successful rehabilitation of the patient.

Sin embargo, pueden ocurrir complicaciones transquirúrgicas, siendo la perforación de la membrana de Schneider la más común. En la literatura se describen varias maniobras en el manejo de las perforaciones de la membrana sinusal, que permiten la reparación y ejecución del procedimiento de injerto en el mismo momento quirúrgico. Entre las formas de reparación, el uso de membranas de Fibrina Rica en Plaquetas y Leucocitos (L-PRF) se ha presentado como una opción de tratamiento con resultados interesantes y prometedores para lograr la reparación. Este artículo describe un caso clínico en el que se reparó la perforación de la membrana de Schneider con membranas L-PRF y el procedimiento se realizó sin complicaciones postoperatorias, lo que permitió la posterior instalación de implantes y la rehabilitación exitosa.A hiperpneumatização do seio maxilar é um fator complicador para a instalação de implantes na região posterior de maxila. O levantamento do assoalho do seio maxilar é uma alternativa para possibilitar a instalação de implantes nestes casos. Entretanto, complicações transcirúrgicas podem ocorrer, sendo a perfuração da membrana de Schneider a mais comum. Diversas manobras são descritas na literatura no manejo das perfurações da membrana sinusal, possibilitando o reparo e a execução do procedimento de enxertia no mesmo momento cirúrgico. Dentre as formas de reparo, a utilização de membranas de Fibrina Rica em Plaquetas e Leucócitos (L-PRF) tem sido apresentada como uma opção de tratamento com resultados interessantes e promissores para se alcançar o reparo.
