Relationship between workload and symptoms during the ergometric test in patients with interstitial lung disease and healthy individuals

Resumo: | Introduction: Interstitial lung diseases (ILD) present similar clinical characteristics such as dyspnea and exercise intolerance. These characteristics are accentuated during physical exertion, although this has not yet been investigated in depth in this population. Aims: To compare the subjective sensation of dyspnea and fatigue by workload during exercise testing (ET) between patients with ILD and healthy subjects. Methods: The individuals performed the CPET on a cycle ergometer with an incremental protocol. At each stage of the test, the sensation of exertion was collected using the Borg scale of dyspnea (D) and fatigue (F). The slopes of the D and F curves by stage in the test were compared using linear regression using the Zar method (F test). Results: 37 patients with ILD and 29 controls were studied. The ILD group had a lower maximum load (60[40-80] vs healthy 120[100-160]; (P<.0001) and greater symptoms at the end of the test for D (P=0.02), but not for F (P=0.23) when compared to the control. Also, the ILD group presented more symptoms per watt (i.e. Borg/Watts) in ET compared to the control group for D (8x10-2±7x10-2 vs 2x10-2 ± 2x10-2; P=0.0007), and F (1x10-1± 9x10-2 vs 3x10-2 ± 1x10-2; P=0.001). D and F symptoms per watt (P<0.05 between curves). Conclusion: Patients with ILD have lower maximal exercise capacity and appear to have a more pronounced increase in D and F sensation per workload during ET. |

Estas características se acentúan durante el esfuerzo físico, aunque esto aún no ha sido investigado en profundidad en esta población. Objetivos: Comparar la sensación subjetiva de disnea y fatiga por carga de trabajo durante la prueba de esfuerzo (PE) entre pacientes con EPI y sujetos sanos. Métodos: Los individuos realizaron PE en un cicloergómetro con un protocolo incremental. En cada etapa de la prueba se recogió la sensación de esfuerzo mediante la escala de Borg de disnea (D) y fatiga (F). Las pendientes de las curvas D y F por etapa en la prueba se compararon mediante regresión lineal utilizando el método de Zar (prueba F). Métodos: Los individuos realizaron el PE en cicloergómetro con protocolo incremental. En cada etapa de la prueba, se recogieron las escalas de Borg de disnea (D) y fatiga (F). Las pendientes de las curvas D y F por etapa en la prueba se compararon mediante regresión lineal utilizando el método de Zar (prueba F). Resultados: se estudiaron 37 pacientes con EPI y 29 controles. El grupo EPI tuvo menor carga máxima (60[40-80] vs sano 120[100-160]; (P<0,0001) y mayor sintomatología al final del test para D (P=0,02), pero no para F (P=0,23) en comparación con el control. Además, el grupo EPI presentó más síntomas por vatio (i.e. decir, Borg/Vatio) en PECP en comparación con el grupo control para D (8x10-2±7x10-2 vs 2x10-2 ± 2x10-2; P=0,0007) y F (1x10-1± 9x10-2 vs 3x10-2 ±1x10-2; P=0,001) Síntomas D y F por vatio (P<0,05 entre curvas). Conclusión: Los pacientes con EPI tienen una capacidad máxima de ejercicio más baja y parecen tener un aumento más pronunciado en la sensación D y F por carga de trabajo durante la PE.Introdução: As doenças pulmonares intersticiais (DPI) apresentam características clínicas semelhantes caracterizadas por dispneia e intolerância ao exercício. Métodos: Os indivíduos realizaram o TE em cicloergômetro com protocolo incremental. A cada estágio do teste foram coletados a sensação de esforço pela escala de Borg de dispneia (D) e fadiga (F). As inclinações das curvas de D e F por estágio no teste foram comparadas por meio de regressão linear utilizando o método Zar (teste F). Resultados: Foram estudados 37 pacientes com DPI e 29 controles. O grupo DPI apresentou menor carga máxima (60[40-80] vs saudáveis 120[100-160]; (P<.0001) e maiores sintomas ao final do teste para D (P=0,02), mas não para F (P=0,23) quando comparados ao controle. Ainda, o grupo DPI apresentou mais sintomas por watt (i.e. Borg/Wattts) no TE comparado ao grupo controle para D (8x10-2±7x10-2 vs 2x10-2 ± 2x10-2; P=0,0007) e F (1x10-1± 9x10-2 vs 3x10-2 ± 1x10-2; P=0,001). A inclinação das curvas Borg/watts diferiu entre os grupos, com DPI apresentando mais sintomas de D e F por watt (P<0.05 entre as curvas).
