Avoidable less than one year old infants deaths in Teresina from 2008 to 2018

Resumo: Avoidable deaths are preventable deaths due to effective actions by health services that are accessible to the population. It is a sensitive indicator of the quality of health care. This study investigated 2,319 live birth deaths occurring in children under one year of age, born to mothers living in the city of Teresina, from 2008 to 2018. These variables were analyzed: Basic cause of death in the classification of preventable cause, ill-defined cause and cause not clearly avoidable. 72% of deaths were classified as preventable, 1% as ill-defined causes and 27% as not clearly preventable. The preventable infant mortality rate decreased from 17.79 deaths per 1000 LB in 2008 to 13.78 in 2018, showing a 27% reduction in cases. The subset of deaths reducible by adequate care for women during pregnancy in the early and late neonatal components predominated, and in the post-neonatal component the causes reducible by diagnostic actions and adequate treatment and reducible by health promotion actions linked to health care actions predominated. During the evaluated period, a reduction in the infant mortality rate was observed, however, there was also a high proportion of preventable deaths, suggesting a challenge to the organization of the three levels of health care.

Se investigaron 2.319 muertes de nacidos vivos en niños menores de un año, hijos de madres residentes en la ciudad de Teresina, de 2008 a 2018. Se analizaron las variables: Causa base de la muerte en la clasificación de causa evitable, causa mal definida y causa no claramente evitable. El 72% de las muertes se clasificaron como evitables, el 1% como causas mal definidas y el 27% como no claramente evitables. La tasa de mortalidad infantil prevenible se redujo de 17,79 muertes por 1000 NV en 2008 a 13,78 en 2018, mostrando una reducción del 27% en los casos. El subgrupo de muertes reducibles por la atención adecuada a la mujer durante el embarazo predominó en los componentes neonatal temprano y neonatal tardío, y en el componente posneonatal, causas reducibles por acciones de diagnóstico y tratamiento adecuado y reducibles por acciones de promoción de la salud vinculadas a acciones de atención. Durante el período evaluado, se observó una reducción en la tasa de mortalidad infantil, sin embargo, también se observó una alta proporción de muertes evitables, lo que sugiere un desafío para la organización de los tres niveles de atención en salud.Mortes evitáveis são óbitos preveníveis por ações efetivas dos serviços de saúde que estejam acessíveis à população. É um indicador sensível à qualidade da atenção à saúde.  Foram investigados 2.319 óbitos de nascidos vivos ocorridos em menores de um ano de idade, filhos de mães residentes no município de Teresina, no período de 2008 a 2018. Foram analisadas as variáveis: Causa básica do óbito na classificação de causa evitável, causa mal definida e causa não claramente evitável. 72% dos óbitos foram classificados como evitáveis, 1% como causas mal definidas e 27% como não claramente evitáveis. A taxa de mortalidade infantil evitável reduziu de 17,79 óbitos por 1000 NV em 2008 para 13,78 em 2018, apresentando uma redução de 27% nos casos. Predominou o subgrupo de óbitos reduzíveis por adequada atenção à mulher na gestação nos componentes neonatal precoce e neonatal tardio, e no componente pós-neonatal predominaram as causas reduzíveis por ações de diagnósticos e tratamento adequando e reduzíveis por ações de promoção à saúde vinculadas a ações de atenção.
