Vulnerability of the gay population living with HIV: an integrative review

Resumo: The current study had the aim of identify the main vulnerabilities of the GAY population living with HIV (Human Immunodeficiency Virus). It was realized an integrative proofreading of scientific articles indexed in the first semestre of 2020, on the databases of Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs) and on the electronic library of Scientific Electronic Library On-line (SciELO), applying the descriptors: HIV AND GAY AND VULNERABILITY. The scientific Productions included approach the different vulnerabilities faced by the gay population, in the three categories: individual, social and programmatic. The United Nations‘ (UN) data point out a prevalence of 21% in the cases of infection by HIV in Brazil between the years of 2008 and 2018. Such resurgence is observed in the GAY and MSM population (Men who have sex with men), which in 2007 represented 32% for 54% in 2017, with the majority of infected in the population of 15 until 29 years old, 1% em 2009 for 18,4% in 2019. The factors that corroborate for the increase of vulnerability are precocius sexual life, low adherence of condom, the difficulty to access health centers, discrimination, stigma and prejudice. It is important to emphasize that the insufficient availability of government resources retards the efforts to provide essential sevices and specific updates on the public policies to this population, contributing even more to the prevalence of vunerability. It is concluded that the study enabled synthesize the findings related to the main vunerabilities faced by the population, on the three categories and the nursing in conjuction with public authorities, have significant influence on the decrease of the vulnerabilities.

Se realizó una revisión integradora de artículos científicos indexados en el primer semestre de 2020, en las bases de datos de la Literatura Latinoamericana y Caribeña en Ciencias de la Salud (Lilacs) y en la biblioteca electrónica Scientific Electronic Library On-line (SciELO), aplicando los descriptores: VIH, GAY Y VULNERABILIDAD. Las producciones científicas incluidas abordan las diferentes vulnerabilidades que enfrentan la población gay, en las tres categorías individual, social y programática. Datos de las Naciones Unidas (ONU) indican una prevalencia del 21% en casos de infección por VIH en Brasil entre los años 2008 hasta 2018. El mencionado incremento se observa en la población GAY y HSH (Hombres que tienen Sexo con otros Hombres), que en 2007 representó 32% hasta 53,4% en 2017, con el mayor número de infectados en la población de 15 hasta 29 años, 1% en 2009 hasta 18,4% en 2019. Los factores que contribuyen para el aumento de la vulnerabilidad son la actividad sexual precoz, baja adherencia al uso del condón, dificultad para acceder a los servicios de salud, discriminación, estigma y prejuicio. Es importante señalar que la baja disponibilidad de recursos gubernamentales impide los esfuerzos para ofrecer servicios esenciales y actualizar políticas públicas específicas para esta población, contribuyendo aún más a la prevalencia de la vulnerabilidad. Se concluye que el estudio permitió sintetizar los hallazgos relacionados con las principales vulnerabilidades que enfrenta la población, en las tres categorías. Los profesionales de enfermería, junto con los poderes políticos, tienen una fuerte influencia en la reducción de las vulnerabilidades presentadas.O presente estudo teve como objetivo identificar as principais vulnerabilidades da população GAY vivendo com HIV (Vírus da Imunodeficiência Humana). As produções científicas incluídas abordam as diferentes vulnerabilidades enfrentadas pela população gay, nas três categorias individual, social e programática. Dados da Organização das Nações Unidas (ONU) apontam uma prevalência de 21% nos casos de infecção por HIV no Brasil entre os anos de 2008 a 2018. Tal recrudescimento é observado na população GAY e HSH (Homens que fazem Sexo com outros Homens), que em 2007 representava 32% para 53,4% em 2017, com maior número de infectados na população de 15 à 29 anos, 1% em 2009 para 18,4% em 20019. Os fatores que corroboram para o aumento da vulnerabilidade são vida sexual precoce, baixa adesão ao uso do preservativo, dificuldade de acesso aos locais de saúde, discriminação, estigma e preconceito. Importante ressaltar que, a disponibilidade insuficiente de recursos governamentais impede os esforços para prover serviços essenciais e atualização de políticas públicas específicas para essa população, contribuindo ainda mais para a prevalência da vulnerabilidade.
